A review of contributing factors indicated that what was thought to be an effective work order system failed because: 1) The WO was written by an engineer based on the Inspection report and not a field check. 2) The polymerization foreman signed off because the storage area foreman was busy elsewhere. 3) A mechanic rather than an instrument man performed the work. 4) A very large section of line was removed In one piece without taking off the guard In an attempt to save time. 5) The maintenance foreman was out and a lead mechanic was supervising. 6) The valve was not fail safe, air to open, but air to close.
