A word of explanation is necessary with regard to Ventilation studies and their inter pretation. There is probably no other test of function in Medicine which is as energy dependent as is Ventilation testing. It is subject to great error. It depends upon the voluntary cooperation of the examinee, who must put forth his best effort, and it is also dependent on the knowledge and experience of the examiner who does the testing, to be able to determine when the examinee is putting forth his best effort, and when the resulting record is a representative one. It is apparent that there is substantial differences between the 1973 and 1974 records ventilation-wise, and this is because of an evident change in equipment. 1973 records were a manual operated .device, 1974 records an electronic device. Experience and the best information available would rate the manual device ahead of the electronic device from the standpoint of accuracy, calibration, and reproducibility of test results. The following personnel appeared to have entirely normal studies, and normal chest x-rays. As indicated, the values demon strated on Ventilation may vary from year to year, but this is a reflection of the comments made in the fore part of this report. Those workmen who have entirely normal chest x-rays and evident normal Ventilation studies for the years of 1973 and 1974 are the following: ~ k (age 20) -: . > (age 47) (for the year 1973 only) (age 19) Marvin l. (age 44) There is a variation in his chest x-rays from 1973 to 1974, The former is reported as normal, the latter has evidence of old inflammatory disease. Ventilation studies are probably satisfactory, although there is again substantial difference between the years of 1973 and 1974. *= - Chest x-rays are satisfactory for both years. There is a minimal defect in Ventilation, but in the over-all this is probably satisfactory. T - - (age 37) X-rays are satisfactory for both years. He has a mild Ventilation defect which is persistent for both years. . - - He has normal x-rays for both years. The values on Ventilation are inconsistent. He has evidence of mild airway obstructive disease, but it is probably not disabling. (age 49) - X-rays demonstrate old healed disease. Ventilation studies demonstrate mild restrictive and airway ob structive disease, probably mildly disabling. - - . ^ ^ (age 51) - Chest x-rays are satisfactory, but there is a great differential between the Ventilation studies for both years, and I suspect that no real difficulty exists. Marvin L. Y. 14207 IBamsiin-UH to - Georgia Pacific (age 35) Chest x-rays are satisfactory. There is a modest defect demonstrated with a great variation between the 1973 and the 1974 studies. I would suspect that this man has mild airway obstructive disease, but again not disabling. " (age 40) Chest x-rays are quite satisfactory. With regard to Ventilation studies, the 1973 value indicates poor function, but 1974 are even worse. I suspect the difficulty here may well be in the accuracy of the test, but I would regard this 1973 value as probably being closer to actual state of Ventilation, and that a modest defect exists. t 'age 59) Chest x-rays are satisfactory. Both 1973 and 1974 demonstrate modest defects inrestrictive disease and airway obstructive disease. The same defect is present for both years. They differ only in the degree of defect demon strated. y --(age 28) Chest x-rays are satisfactory. Again the difference in Ventilation studies here really demonstrates the energy dependency of the test, and I suspect that there is really nothing wrong with the examinee. ----- (age 49) Chest x-rays are quite satisfactory. The 1974 values seem quite satisfactory. I suspect the 1973 values were either the result of poor cooperation or lack of diligent testing. . (age 58) Chest x-rays show pulmonary fibrosis and emphysema. Ventilatbn studies have substantial evidence of both airway obstruction and restrictive disease, and these are compatible with the chest x-rays. This man probably has significant disability. . * (age 57) Chest x-rays are satisfactory. He probably has chronic bronchitis and some emphysema, since Ventilation studies do demonstrate some restrictive and airway obstructive disease. Marvin L. Amour. M. D. Robert a. Caruti. One would never recognize them as having been done upon the same examinee. I would suspect that either there is no defect, or a very mild defect is present. I trust this summary will facilitate your interpretation of these reports. I would personally regard Mr. McCarger tp^ffe the only examinee with very substantial problems.
