Failure of a level control valve compounded by a faulty tank overflow alarm resulted in the overflow of alkaline material exceeding capacity of the pH control system. The level control valve and overt alarm were both repaired on 12/10/84. 1 (7 min.) The pH'-control system became* overloaded when several operatibnal events occurred simultaneously. In addition, adequate neutralization was hindered by weak caustic reagent. - (1 hr. 11 min.) The SOP involving the sampling/analysis of cooling wash water before draining to the Drocess sewer was not'followed; the coolino wash water was hiaher in alkalinity than expected causinq neutralization capacity to be exceeded.
