We have not yet been notified that a formal Application has been filed with the Illinois Industrial Commission. However, I assume that it is merely a matter of time before this is done. Please protect the interests of Sherwin-Williams in this matter. According to the First Report of Injury, alleges that he fell off a trailer and sustained injury to his right shoulder, right elbow, right thigh, and lower back. As you can see from Hr. Colangelo's October 5, 1989 correspondence, Sherwin-Williams does not wish to pay any benefits for this alleged claim. I have set an appointment for to be seen by Dr. Dwyer on October 27, 1989. Upon my receipt of his report, I will forward a copy to you. However, please note that called our office and advised that he did not know if he would be keeping this appointment. He stated that he was being released from his own physician on 10/23/89 and he did not wish to lose any time from work at his "new" job. Apparently, he is employed elsewhere at this time.
