Package - Polyethylene caution bags with the following message printed on each bag - "Caution; contains asbestos fibers; avoid creating dust; breathing asbestos dust may cause serious bodily harm." Sealed bags must be shipped via closed truck used exclusively for consignor and shipped only to consignee. b. Labeling - No DOT labeling required. c. Marking - Mark each package in letters at least h inch high "Waste asbestos". Also mark "ORM-C" in letters at least H inch high and surrounded by a rectangle at least h inch away from the "ORM-C" designation on all sides; this marking must follow or be placed below the name "Waste asbestos" on each package. d. Placarding - No DOT placarding required. e. Shipping Papers - The basic description to be used on shipping papers is as follows: "Waste asbestos, ORM-C". Shipping papers must reflect the total quantity either before or after this basic description. The following CERTIFICATION statement must be made and signed on the shipping papers: "This is to certify that the above^Tor herein-) named materials are properly classified, described, packaged, marked and labeled and are~~in proper condition for transportation according to the applicable regulations of the Department of Transportation". II. EPA Manifest Requirements - the manifest must contain: a. A manifest document number. b. The generator's name, mailing address, telephone number, EPA ID number, handwritten signature and date. c. The name and EPA ID number of the designated facility; the name and EPA identification number of each transporter; and the handwritten signature of the initial transporter and date of acceptance. d. The description of the waste - "Waste asbestos, ORM-C". e. Total quantity of waste materials; type and number of containers on transport vehicle. f. The following certification must appear on the manifest"This is to certify that the above named materials are properly classified, described, packaged, marked, and labeled and are in proper condition for transportation according to the applicable regulations of the Department of Transportation and the EPA".
