How many producers have an emergency vent on each reactor? Also, is steam induced into the vent while depressuring and during the emergency? 2. Do any of the producers use flares for emergency venting? 3. Are jumper grounds attached before opening screwed joints and/or connections when monomer has not been purged from equipment? 4. Are permanent jumper grounds installed at plug type valves, ball type valves and flanged joints in monomer service? 5. Do any producers use plastic pipe and/or plastic tubing for transferring materials to reactors? 6. A case history of accidents producers have experienced involving lauryl peroxide and/or IPP. 7.
