The question of signifi cant exposure is one that will need to be answered by our industrial hygienists. The Instructions are designed primarily for Ethyl designated physicians who will need to send their reports to the Corporate Medical Office. This does not apply in your case as the medical files will be kept in your office. However, because of the legal implications, the Corporate Medical Office should be informed of any suspected or con firmed cases of asbestosis. Thank you for your usual cooperation. Insulators or others with potentially significant asbestos exposure. 2. History of six months or more of potentially signifi cant asbestos exposure in the past. 3. Physical examination or x-ray findings which indi cate the possibility of past asbestos exposure. 4. Industrial hygiene data which indicates the work place has the potential for significant exposure. Eligibility for examination at Company expense is determined by the Ethyl Medical Department.. MEDICAL REQUIREMENTS: 1. A complete history and physical examination on Ethyl Form CM 7801, including core clinical laboratory work. 2. A 14 x 17 PA and lateral chest x-ray with detailed radiological interpretation of the state of the pleura, and a statement regarding the presence or absence of pleural abnormalities such as thickening, calcification, plaque, effusion, etc. including a detailed description of the lung parynchema. 3. Pulmonary function tests: Forced vital capacity (FVC) Forced expiratory volume at one second (FEV^) 4. A review by the Ethyl Medical Department and, when indicated, a special examination by a consultant who is a recognized specialist in pulmonary disease. The consultant must be cognizant of the manifesta- ... tions of pneumoconiosis and, in particular, of asbestosis. EXAMINATION INTERVAL: 1. A history of exposure only; no laboratory or clinical evidence of asbestosis. Examination reveals minimal to moderate lateral pleural wall thickening, minor parynchemal lung changes, slight decrease in pulmonary function reflective of restrictive lung disease. Any or all may be present. Examination interval: 6-12 months 3. Examination reveals the presence of pleural plaques, calcified pleural plaques involving lateral wall or diaphragmatic pleura, significant pulmonary paryn chemal changes, pulmonary function descrease of FVC < 80% or FEVj_ < 70%, adjusted for race. Examination interval: 6 months OTHER REQUIREMENTS: 1. Pre-placement examination within 30 days of asbestos assignment. 2. Termination examination within 30 days before or after termination of employment. 3. Medical records must be retained at least 20 years. PATIENT COUNSELING: The examiner should inform smokers that smoking signifi cantly increases the risk of lung cancer in workers exposed to. asbestos. ELIGIBILITY: 1. Employees engaged in work ' involving levels of asbestos, et al (tremolite, anthopylite, actinolite or a combination of these minerals) at or above the action level (0.1 fiber/cc air) for 30 or more days per year. 2. Employees required to wear negative pressure respirator because of exposure to asbestos et al. 3. Initial annual and termination examination. MEDICAL REQUIREMENTS: 1. Initial examination: Have examinee complete an Initial Medical Questionnaire. 2. Reexamination: Have examinee complete a Periodic Medical Questionnaire. 3. A complete history and physical, examination with special emphasis on the pulmonary and gastrointestinal systems on Ethyl Form CM 7801. 4. Clinical laboratory screen including urinalysis and CBC. 5. A 14 x 17 PA and lateral chest x-ray with detailed radiological interpretation of the state of the pleura, and a statement regarding the presence or absence of pleural abnormalities such as thickening, calcification, plaque, effusion, etc. including a detailed description of the lung parynchema for initial examination purposes. Periodic examination chest x-rays are performed at the discretion of the examining physician. 6. Pulmonary function tests: Forced vital capacity (FVC) Forced expiratory volume at one second (FEV.) 7.
