A1 and I will coordinate the necessary investigation to defend this claim. I am enclosing a copy of the Employer's Report form 5020 that I received from the Fresno store indicating a date of injury of 5-12-93. The applicant's attorney is alleging injury for 3-31-93 which I have no record. I am also enclosing the only medical reports I have. I have responded to applicant's attorney's Request for Information form and have requested that he have his client fill out and return Employee Claim forms for these two injuries. When the loss came in for 5-12-93,1 spoke to the store and was told) is helping a customer paint a car. He wore the proper mask and hood, but evidently was exposed to some pain fumes. He was off work from 5-13-93 to 5-18-93 and returned to work on 5-19-93. He has since quit and is working for another company.
