This inspection report may also include information gathered from a review of EPA, State and/or public records. SECTION I - INTRODUCTION Facility Description The Princeton-Hampton Wastewater Treatment Plant (hereinafter referred to as the "Facility") is a wastewater treatment facility located along State Highway 6 in Princeton, Idaho, within the northwestern portion of Latah County. The Facility collects and treats domestic wastewater for a resident population of 175 living within the unincorporated communities of Princeton and Hampton. The Facility is owned by the Princeton-Hampton Water & Sewer District (District) and is currently operated by contracted operator Mr. Elmer Johnson. Mr. Johnson has been employed by the District since January 2024. The Facility was constructed in 1981 and was engineered/designed by Hamilton & Voeller. The design flow is approximately 0.02 million gallons per day (MGD). The collection/treatment system is generally composed of a gravity fed sewer line with two lift stations and attached force mains. Wastewater treatment occurs within a two-cell facultative lagoon system, followed by tablet chlorination. Treated/disinfected wastewater is then batch discharged intermittently to the Palouse River. The discharge location is approximately one quarter mile upstream from the confluence of the Palouse River and Hatter Creek. See Attachment A, Facility Map. Permit Information The Facility is permitted to discharge treated wastewater to the Palouse River under the Idaho Department of Environmental Quality (IDEQ) NPDES Individual Permit ID0028436. The District initially applied for NPDES permit coverage on July 22, 2008 (CWA-2012-0123), through EPA Region 10 (EPA). On August 6, 2010, EPA determined the district's application was complete following the receipt of supplemental information on design flow. A subsequent compliance order was issued by EPA on May 4, 2012, requiring influent/effluent monitoring, and reporting until a permit could be issued. Permitting Authority transferred from U.S. EPA Region 10 to IDEQ as part of the July 2018, Phase I NPDES Transfer. IDEQ issued the current permit, effective April 1, 2020, and due to expire on March 31, 2025. Since receiving permit coverage, the Facility has only reported discharges in 2021 and 2024. During times of discharge, effluent monitoring is required from the Facility's outfall (Outfall 001) to the Palouse River and influent monitoring is required from the Facility's influent lift station (Lift Station 1), located approximately one quarter mile from the lagoon system. Effluent parameters include Total Suspended Solids (TSS), Biochemical Oxygen Demand (BOD), E. Coli, pH, Total Residual Chlorine (TRC), Flow, Total Ammonia (as N) and Temperature. Influent monitoring parameters include Flow, BOD, TSS. The permit also requires reporting of received septage (gallons), quarterly receiving water monitoring above the influence of the Facility's discharge, and a compliance schedule for TRC. Inspection/Compliance History The Facility was last inspected for NPDES compliance on November 16, 2021, by IDEQ. Findings included expired operator licenses, failure to submit Discharge Monitoring Reports (DMRs), failure to submit schedule submission items and failure to conduct receiving water monitoring. IDEQ issued a Notice of Violation (NOV) with penalties on April 4, 2022. A Consent Order was issued by IDEQ on April 24, 2023. See Attachment B, IDEQ Consent Order Case No. ID2022-0027. Page 2 of 47 2024 CWA NPDES ID0028436 Inspection Report On July 19, 2023, IDEQ approved a milestone extension request under the Consent Order for the requirement to apply for funding requirement through the USDA Rural Development Grants by July 1, 2023. The deadline was extended to September 30, 2023. On July 30, 2024, IDEQ approved a second milestone extension request under the Consent Order for the requirement to develop and submit completed Preliminary Engineering Report by July 1, 2024. The deadline was extended to October 1, 2024. Entry and Inspection Chronology This was an announced inspection by EPA Region 10; IDEQ was invited to participate in the inspection but was not in attendance. On July 23, 2024, I spoke with Mr. Elmer Johnson, Contracted Operator for the PrincetonHampton Sewer District. During the phone call, I discussed that I had been asked to conduct an on-site Clean Water Act inspection at the Facility to assess compliance with the IDEQ issued NPDES permit. Mr. Johnson welcomed the inspection, and we agreed on an inspection date of July 31, 2024. To assist in coordinating with others as needed, I followed up with an email to Mr. Johnson and Ms. Jessica Lusby, Princeton-Hampton Sewer District Board President. In my email I restated the purpose and expectations of the inspection and included information regarding EPA's Technical Assistance Webinar Series for "Improving CWA-NPDES Permit Compliance." On the day of the inspection, I arrived at the front access gate to the lagoons at approximately 2:00PM. I was greeted by Mr. Johnson as well as Princeton-Hampton Sewer District Board Representative, Mr. Curry Ely. We had initial introductions; I presented my EPA inspector credentials, and I restated the purpose and scope of the inspection. Mr. Johnson briefly discussed recent activities/work projects at the Facility since becoming the Facility's operator. Among others, activities included addressing electrical and pump/float improvements at the Facility's two lift stations as well as scheduling smoke testing a portion of the collection system. It was discussed that Mr. Ely has assisted Mr. Johnson in various maintenance activities as needed, including the recent construction/installation of new lagoon weir gates. This inspection consisted of an opening conference, Facility walk-through, records review and concluded with a closing conference. For the Facility walk-through, I observed the various components of the wastewater treatment system, including the two lift stations, septage receiving station, two-cell lagoon system, chlorine contact structure and outfall. Photographs taken during the Facility walk-through are attached to this inspection report as Attachment C, Photograph Log. During the closing conference I discussed my observations, potential compliance concerns and next steps, including the inspection report and compliance review process. I was accompanied throughout the inspection by Mr. Johnson and Mr. Ely. SECTION II - OBSERVATIONS Site Review The Facility generally operates as wastewater gravity flows from the District's sanitary sewer collection system into two lift stations. The collection system consists of approximately 8,590 lineal feet of 8-inch diameter PVC pipe and 20 concrete manholes. A primary influent lift station (Lift Station 1) is located east of the lagoons and serves most of the collection system, approximately 77 homes. Lift Station 1 includes two submersible sewage pumps controlled by floats and discharges to a four-inch diameter forcemain which then has three discharge locations/options within the two-cell lagoon system. The second lift station (Lift Station 2) is in the southeast corner of the District's service area and serves approximately 10 homes. Lift Station 2 is operated by two submersible sewage pumps controlled by floats and discharges to the forcemain and is received at Lift Station 1. The Facility's two-cell lagoon system includes Lagoon 1 and Lagoon 2. Each lagoon can store approximately 2.28 million gallons at maximum operational depth. Raw wastewater is pumped into Lagoon 1 at two submerged locations, in the NE and NW corners of the lagoon. A septage receiving station is also located along the north end of Lagoon 1, as a local septage hauler is charged for intermittent septage discharges into the system. Page 3 of 47 2024 CWA NPDES ID0028436 Inspection Report Wastewater flows from Lagoon 1 to Lagoon 2 through a 10-inch diameter PVC pipe located in the SW corner of Lagoon 1. The Lagoon 2 outlet is in the SE corner of the lagoon which discharges to an earthen chlorine contact basin via a 6-inch diameter pipe. If needed, a 6-inch diameter outlet on Lagoon 1 allows for bypassing of Lagoon 2. Lagoon 1 can also be bypassed where influent directly enters Lagoon 2 via valve design. Disinfection occurs as wastewater from Lagoon 2 is routed to a mixing box upstream of the earthen chlorine contact basin that contains Calcium Hypochlorite tablets. Effluent can then be discharged as needed via removable stop gates to a slough of the Palouse River via a 10-inch diameter PCV pipe. In 2021, the district contracted with Mountain Waterworks for the development of a "Wastewater Facilities Plan." The Plan identified that although functioning, the mechanical life of both lift station pumps had been exceeded and the floats were also in need of replacement. The Plan recommended several upgrades to the lift stations, disinfection system and overall treatment facility. The Plan was submitted on March 15, 2022, and was approved by IDEQ on April 15, 2022. The Facility has since contracted with Merrick & Company in Lewiston, Idaho for the development of a Preliminary Engineering Report associated with the suggested wastewater improvements. As discussed earlier in this report, since becoming the Facility's contracted operator in January 2024, Mr. Johnson started/completed several projects designed to improve the district's collection and treatment system. These projects included addressing the electrical and pump/float improvements at both lift stations, lagoon weir gate replacements, and scheduling smoke testing for a portion of the collection system. Mr. Johnson discussed that he maintains a maintenance log documenting his actions since becoming the contracted operator. See Attachment D, Princeton-Hampton Sewer District Maintenance Log. Mr. Johnson discussed that there had been no prior maintenance records or operator logs to his knowledge. It was discussed/reported that the Facility has had only one discharge event while under the operation of Mr. Johnson. The planned discharge occurred in June 2024, in coordination with IDEQ. Walk-Through Observations: Location: Lagoon 1, Lagoon 2, Chlorine Mixing Box, Chlorine Contact Chamber, Outfall 001 Observation #: OB-001 Following my initial introductions with Mr. Johnson and Mr. Ely, we began the walk-through portion of the inspection, generally following the process flow. We started the tour near the main entrance gate, north of Lagoon 1 and west of a storage building (Photo 1). It was discussed that wastewater from the district's primary lift station (Lift Station 1), located approximately one quarter mile away, enters Lagoon 1 via two submerged locations. One location in the NW area of the lagoon and one in the NE area of the lagoon. Mr. Johnson discussed that the valve locations are identified on Facility blueprints, but the NE connection was only recently viewable after the area was mowed earlier in the month. It was discussed that Lagoon 1 can also be bypassed where influent directly enters Lagoon 2 via a valve (currently closed) located southwest of Lagoon 1. We then observed the septage receiving station located along the north bank of Lagoon 1 (Photo 2). Mr. Johnson discussed that the district maintains a septage receiving log for the local septage hauler. Received septage is then calculated and reported on the monthly DMR during months of discharge. We continued to the south side of Lagoon 1 and observed the lagoon transfer structure to Lagoon 2 (Photo 3), allowing for wastewater to pass from Lagoon 1 to Lagoon 2. It was discussed that in May 2024, Mr. Ely had assisted in the construction and installation of 10 new weir gates to help manage lagoon levels, as many were found missing when Mr. Johnson first started as the operator. Lagoons levels were observed to be relatively low, following the June 2024 discharge event (Photo 4, Photo 5). Page 4 of 47 2024 CWA NPDES ID0028436 Inspection Report We continued to the southeast end of Lagoon 1 and observed the Lagoon 2 bypass structure that routes wastewater from Lagoon 1 directly to the chlorine mixing box (Photo 6). We then walked further south and observed the Lagoon 2 discharge location into the chlorine mixing box (Photo 7), and the chlorine mixing box (Photo 8). The mixing box was dry/empty at the time of inspection, Mr. Johnson discussed that he had replaced the box containing the chlorine tablets earlier in the year to try and achieve better contact time. The Facility utilizes Accu-Tab Calcium Hypochlorite tablets for chlorination. Tablets are placed within a homemade bucket with large holes drilled around it for dosing (Photo 9, Photo 10). We then observed the earthen chlorine contact chamber and overflow outlet structure (Photo 11 - Photo 13). It was discussed that historically, the Facility has experienced issues with the current design, as the Palouse River can backflow into the chlorine contact basin when experiencing higher/seasonal water levels within the river. We continued to the south and observed Outfall 001 within a slough of the Palouse River (Photo 14). The outfall area was overgrown with grass, Mr. Johnson discussed that the area could be improved to ensure safe sample collection. We continued back towards the Facility's storage building, Mr. Johnson discussed one of proposed engineered re-designs which would include an earthen area east of the lagoons to be used for the construction of a chlorine contact chamber (Photo 15). I also observed various Facility blueprints within the storage building, and Mr. Johnson discussed/displayed that he utilizes a Model C-900 Chlorine Colorimeter for field analysis of residual chlorine. All other parameters were discussed to be analyzed by Anatek Labs, Inc. I discussed with Mr. Johnson that per the standard method, pH analysis is considered a field measurement has a maximum hold time of 15 minutes. It was discussed that quarterly upstream/receiving water monitoring occurs along the Palouse River, at a bridge located approximately 0.3 miles south of the intersection and of Hatter Creek Road and Highway 6. Location: Lift Station 1 & Lift Station 2 Observation #: OB-002 We left the gated lagoon area and drove approximately one quarter mile to the east before entering an area north of the railway. We observed the primary lift station (Lift Station 1) (Photo 16, Photo 17), it was discussed that Lift Station 1 is located on gated private property and the district has access permission. As discussed earlier in this report, Lift Station 1 includes two submersible sewage pumps controlled by floats and discharges to a four-inch diameter forcemain. Mr. Johnson discussed that during some electrical troubleshooting this summer, it was discovered that half of the floats were not working and were programmed incorrectly. Mr. Johnson also found them to be programmed to both run at the same time, attributing to one pump doing most of the work. In June 2024 and July 2024, a new Flygt 3085 pump was installed within the lift station, floats were corrected, and wiring was fixed. It was discussed that flow is calculated by the pump hours off the pump meter display (Photo 18). Influent flow is approximately 20,000 gallons-per-day (GPD). Mr. Johnson discussed that smoke testing was scheduled for August 28, 2024, in coordination with Idaho Rural Water Association. We walked back to our vehicles and then travelled to Lift Station 2 (Photo 19, Photo 20) located in the southeast corner of the district's service area within a small residential area. As discussed earlier in the report, Lift Station 2 includes two submersible sewage pumps controlled by floats and discharges to a four-inch diameter forcemain. It was discussed that the lift station was pumped out in June 2024, and a new pump was installed. The walk-through concluded after we observed the lift station and the electrical improvements that were made in 2024, including moving the electrical junction box outside of the wet well. Page 5 of 47 SECTION III - Records Review 2024 CWA NPDES ID0028436 Inspection Report The following records were reviewed as part of the inspection: Record: Discharge Monitoring Reports (DMRs) & Monitoring Data Ref #: RR-001 Prior to the inspection, I reviewed monthly DMR's that were submitted since the April 4, 2022, Notice of Violation by IDEQ. This was also completed by a review of a 5-Year exceedance report generated by EPA's Integrated Compliance Information System (ICIS). As discussed earlier in this report, the Facility's has had one reported discharge since 2021, the discharge occurred in June 2024, in coordination with IDEQ. According to Mr. Johnson and the District Maintenance Log (Attachment D), the June 2024 discharge event began on June 5, 2024, discharging approximately 5 GPM according to Mr. Johnson's estimate. Effluent flow was then adjusted by Mr. Johnson to approximately 2-3 GPM on June 10, 2024, and then ceased on June 13, 2024. The review noted that as discussed in the permit, for months not discharging influent/effluent parameters on the corresponding DMR are reported with a No Data Indicator (NODI) Code "C" indicating "No Discharge." The review noted that monthly DMRs for June 2022, March 2023, and September 2023 were received after the permit required deadline (20th of the month following the completed reporting period). See Attachment E, 5Year ICIS DMR Non-Receipt Violation Report. The review noted that monthly influent/effluent DMRs for April 2022 and May 2022 were observed to be not received/submitted. Quarterly receiving water monitoring DMRs for the 2nd and 3rd quarter of 2022 were also observed to be not received/submitted. See Attachment E, 5-Year ICIS DMR Non-Receipt Violation Report. The review noted that results from the June 2024 sample events exceeded effluent limits for the weekly average of TSS and monthly geometric mean of E. Coli. See Attachment F, ICIS Effluent Exceedance Limit Report. As discussed earlier in this report, at the time of inspection, Mr. Johnson discussed that sample analysis is conducted by Anatek Labs, Inc. for all parameters except Total Residual Chlorine. Analytical records from Anatek Labs, Inc. including chain-of-custody for each sampling event were reviewed. See Attachment G, June 2024 Analytical Results. The review noted that on June 6, 2024, effluent samples were collected and analyzed for TSS, BOD and E. Coli. On June 10, 2024, influent and effluent samples were collected and analyzed for BOD and TSS, as well as effluent samples for pH, Ammonia and E. Coli. On June 13, 2024, effluent samples were again collected and analyzed for TSS, BOD, pH and E. Coli. When reviewing chain-of-custody documentation, it was observed that samples were being documented as received on ice, between 9.8 and 16.8 degrees Celsius. As discussed earlier in this report, received septage is calculated and reported on the monthly DMR for months when discharge occurs. For months when there is no discharge, the corresponding DMR is marked with the NODI Code "C" (No Discharge). For June 2024, the Facility reported 85,820 gallons of received septage. Record: Quality Assurance Project Plan (QAPP) Ref #: RR-002 I requested to review the permit required Quality Assurance Project Plan (QAPP), Mr. Johnson provided an electronic copy of the QAPP via email. The QAPP was prepared by Mountain Waterworks, Inc. and dated Page 6 of 47 2024 CWA NPDES ID0028436 Inspection Report November 2021. Notification to IDEQ was submitted/uploaded on November 26, 2021, by Mr. Richard Hooker, prior District Board President. Upon review, the QAPP was consistent with the permit requirements. It was noted that the QAPP should be updated to include the current laboratory, Anatek Labs Inc. The QAPP lists Accurate Testing in Coeur d'Alene, Idaho as the contract laboratory. Record: Wastewater Facilities Plan Ref #: RR-003 I requested to review the permit required Facility Plan, Mr. Johnson provided an electronic copy of the document titled "Wastewater Facilities Plan" via email. As discussed earlier in this report, the Wastewater Facilities Plan (Plan) was prepared by Mountain Waterworks, Inc. The Plan was dated December 2021, and revised March 2022. The Facility submitted the Plan on March 15, 2022, it was approved by IDEQ on April 15, 2022. The Plan identified that although functioning, the mechanical life of both lift station pumps had been exceeded and the floats were also in need of replacement. As discussed earlier in this report, the Plan recommended several upgrades to the lift stations, disinfection system and overall treatment facility. The Facility has since contracted with Merrick & Company in Lewiston, Idaho for the development of a Preliminary Engineering Report associated with the suggested wastewater improvements. On June 10, 2024, the Facility requested an extension of the Preliminary Engineering Report submittal, required under the Consent Order. Record: Emergency Response Plan Ref #: RR-004 I requested to review the permit required Emergency Response Plan, Mr. Johnson provided an electronic copy of the document titled "Emergency Response Planning Template for Public Wastewater Systems" via email. The document was not dated; however, notification to IDEQ was submitted/uploaded on November 26, 2021. Upon review, it was noted that the Emergency Response plan should be updated to include current Facility personnel and updated contact information. Record: Spill Control and Response Plan Ref #: RR-005 I requested to review the permit required Spill Response Plan, Mr. Johnson provided an electronic copy via email. The document was dated October 2021, and notification to IDEQ was submitted/uploaded on November 26, 2021. Upon review, it was noted that the Spill Response Plan should be updated to include current Facility personnel and updated contact information. Record: Maintenance Log Ref #: RR-006 As discussed earlier in this report, Mr. Johnson discussed that he maintains a maintenance log documenting his actions since becoming the contracted operator. These actions included addressing the electrical and pump/float improvements at both lift stations, lagoon weir gate replacements, and scheduling smoke testing for a portion of the collection system. Mr. Johnson provided a copy of the maintenance log via email. See Attachment D, Princeton-Hampton Sewer District Maintenance Log. Mr. Johnson also provided manufacturer specification and the operation and maintenance manual for the newer Flygt 3085 lift station pumps. Page 7 of 47 SECTION IV- SAMPLING ACTIVITIES AND ANALYTICAL RESULTS No sampling was conducted for this inspection. 2024 CWA NPDES ID0028436 Inspection Report SECTION V - AREAS OF CONCERN The presentation of Area(s) of Concern (AOC) does not constitute a formal compliance determination or violation. The following AOC was identified as during the inspection: AOC Reference #: AOC-001 Effluent Limit Exceedances, Narrative Limits Regulation and/or Permit Requirement: Section 1.2. of the permit states that "the permittee must comply with the effluent limits in Table 2 and Table 3 at all times unless otherwise indicated, regardless of the frequency of monitoring or reporting required by other provisions of this permit." Table 2 and Table 3 includes effluent monitoring limits for Total Suspended Solids (TSS), Biochemical Oxygen Demand (BOD), E. Coli, pH, and Total Residual Chlorine (TRC). Section 1.2.1 of permit states that "the permittee must observe the receiving water once per month in the vicinity of where the effluent enters the surface water. The permittee must maintain a log of each observation that includes photos date, time, observer, and whether there is a presence of floating, suspended or submerged matter, or other indication that the discharge causes a violation of IDAPA 58.01.02.200 narrative criteria. The log must be retained onsite and made available to DEQ upon request." As discussed earlier in this report, based upon my review of the reported 5-year effluent data using EPA's Integrated Compliance Information System (ICIS) and discussions at the time of inspection, the Facility's has had one reported discharge since 2021, the discharge occurred in June 2024, in coordination with IDEQ. Reported results from the June 2024 sample events included effluent limit exceedances for the weekly average of TSS and monthly geometric mean of E. Coli. See Attachment F, ICIS Effluent Limit Exceedance Report. When compared against the permit requirements, no observation log has been maintained. AOC Reference #: AOC-002 Historic Discharge Monitoring Reports Regulation and/or Permit Requirement: Section 2.2.3. of the permit states that "monitoring data must be submitted electronically using NetDMR no later than the 20th of the month following the completed reporting period. As discussed earlier in this report, I reviewed monthly DMR's that were submitted since the April 4, 2022, Notice of Violation by IDEQ. This was also completed by a review of a 5-Year exceedance report generated by EPA's Integrated Compliance Information System (ICIS). The review noted that monthly DMRs for June 2022, March 2023, and September 2023 were received after the permit required deadline (20th of the month following the completed reporting period). Monthly influent/effluent DMRs for April 2022 and May 2022 were observed to be not received/submitted. Quarterly receiving water monitoring DMRs for the 2nd and 3rd quarter of 2022 were observed to be not received/submitted. See Attachment E, 5-Year ICIS DMR Non-Receipt Violation Report. Page 8 of 47 2024 CWA NPDES ID0028436 Inspection Report AOC Reference #: AOC-003 pH Analysis and Sample Preservation Regulation and/or Permit Requirement: Section 2.1.6 of the permit states that "required monitoring must be completed using sufficiently sensitive methods and conducted according to test procedures approved under 40 CFR 136, unless: Another method is required under 40 CFR subchapters N or O; or This permit requires the use of specific EPA approved method for a particular parameter. Within 40 CFR 136, Table II- Required Containers, Preservation Techniques, and Holding Times, the maximum holding time for pH states, "Analyze within 15 minutes," and samples should be preserved/cooled to a temperature of 6 degrees Celsius or less. As discussed earlier in this report, at the time of inspection, Mr. Johnson discussed that sample analysis is conducted by Anatek Labs, Inc. for all parameters except Total Residual Chlorine. I discussed with Mr. Johnson that per the standard method, pH analysis is considered a field measurement and has a maximum hold time of 15 minutes. Regarding field analysis I also discussed the "Recording of Results" section of the permit (Section 2.2.1) to ensure sample collection of field parameters is properly documented.
