No facility representatives were present at the time of the inspection. The inspection was conducted by the authority of the Clean Water Act (CWA) to determine compliance with Environmental Protection Agency (EPA) regulations. There were no other federal, state, or local agency representatives present at the time of the inspection. FACILITY DESCRIPTION EPA conducted an inspection on the LBOC, LLC's Baker Sand Unit 12-2 & Lion-Staples No. 2 Facility, which consists of four (4) storage tanks, one (1) separator, two (2) pumps, and one (1) well. Section II - OBSERVATIONS I observed the facility, associated signage, the pumps, the injection well, and secondary containment with the separator and storage tanks; see Appendix 1 (Photograph Log), Photo No. 1, No. 2, No. 4, No.6, No 8. For additional GPS location and photo metadata, see Appendix 1. I observed a transfer pipe/load line outside of secondary containment with loose oil and no catchment bucket, see Photo No. 3. I observed gaps under one (1) storage tank and loose oil, see Photo No. 5. I observed a leaking pump surrounded by loose oil, see Photo No. 7. I observed an open, uncapped secondary containment discharge valve with loose oil under the valve, see Photo No. 9 through No. 11. I observed impacts of a discharge outside of secondary containment, see Photo No. 12 through No. 15. I observed the well at the facility surrounded by loose oil, see Photo No. 16. Section III - AREAS OF CONCERN No closing conference was held due to no LBOC, LLC staff present during the inspection.
