Box 10385 Jackson, MS 39204 VISTA Dear Mr. Banks: This letter will confirm our conversation of September 10, 1984 at 10:45 a.m. I informed you of a leak that occurred at 11:10 a.m. on September 9, 1984. Prior to reactor D-300 being charged, the reactor was pressured up to about 10 - 15 PSIG with VCM gas in accordance with plant operating procedures. The outside operator checked the reactor and since everything looked normal, he instructed the panel operator to proceed with the charge. The out side operator then walked across the recovery structure and heard a noise that sounded like an air leak. As he walked back to the reac tor, he thought there might be a problem and radioed to the panel operator to stop charge. The panel operator immediately stopped charging on the reactor, noting that no additional VCM was added to the reactor since the initial amount. The outside operator and chief lead operator immediately discovered that the cause of the noise was from the reactor atmospheric vent valve. The valve was immediately closed and the leak stopped. The connector between the actuator and the valve had worn and the valve was slightly cracked open, even though the indicator light on the panel indicated closed.
