In addition, there is a letter introducing the medical monitoring program to examining physicians and should be sent to the designated physician who will be performing the examination for CPD locally. Finally, enclosed are pre-placement questionnaires, periodic questionnaires and forms for reporting the results of these examinations which should also be given to the examining physicians.
