Tromblee / The Safety and Housekeeping Inspection Committee that inspected the Chocolate Bayou Plant on September 17 and 18 have compiled a list of observations that may be useful to us in Texas City. The committee was comprised of Harry Conrad, Ray Forsthoffer, Rob Fransham, Nick Monroe, Mark Riddle and Casey Yauger. Members of the com mittee have agreed to make themselves available to anyone in the plant that may want additional information about a specific item. Our ob servations, listed below, are in no particular order. - "Job Safety Analyses" (JSA) are conducted throughout the plant anytime a job - maintenance or otherwise, is non-routine or requires a special permit. This is a good program and should be considered as a follow-up to the STOP Program. - Red nylon windsocks are used throughout the plant rather than the metal windsocks that are used in our plant. They are more visible and probably require less maintenance. - All safety showers and eye baths are painted a very bright, high visibility, green. - An operator is present at every first break into piping or equipment. - Ladder inspections are required monthly and tag on each ladder indicates the month. - All scaffolds have independent ladders. - A plastic lockout tag which can be written on and erased is used in conjunction with a lock. The tag describes the reason for the lock plus who installed it. LAM014539 SC 13368 STG 2430338 2- - Sign-in boards are used in every unit. Table of chemical hazards, their properties, odor and threshold limit values, personal protective equipment required and first aid procedures are posted on control room walls. - Unit trucks are equipped with seats on both sides, parallel to the truck body, with a rail at the end of the seats. Tailgate is removed from the truck and a step-up bumper is provided. Flooring of truck has non-skid surface and a half camper shell is installed for weather protection. - Unit trucks are fitted with a special rack on the bumper for carrying lethal service samples. Special signs indicate that the vehicle is carrying lethal samples. - Personal protective equipment required on a maintenance job is included on the work order. This provides advanced information to the main tenance foreman and craftsman. - Some amyl nitrate boxes were alarmed. - Heavy duty scissors were fastened to some safety showers. - A portable emergency equipment cart containing vapor proof suits, self-contained breathing apparatus and other emergency equipment was in use in the AN Unit. This allows the operators to move the equipment near the scene of the emergency. - All operators in the new MHBA Unit are required to train with the vapor proof suit and the six minute air bottle on a weekly basis. - One unit had a "hurricane locker" containing all of the necessary itemsrequired during a hurricane, including emergency drinking water. The locker was kept locked and only the shift foreman had a key. - HCN sample cabinets in the NTA/IDA Unit had calibrated fill tubes so that an operator never overfills a sample bottle. - About one-half of the Bi-Annual Loss Prevention Teams are hourly. SC 13369 LAM014590 STG 2430339 - "Safety is an individual responsibility" is emphasized throughout the plant. There is very little reliance on formal safety commit tees and/or task forces. JSA's and incident investigations are conducted by teams which are assembled for each specific case. Their teams always have one or more hourly personnel assigned. - The plant "Emergency Response Team" is made up of only shift workers, therefore, assuring that a team is always readily available. - Each shift in every unit is staffed with at least one EMT, - All operating, safety and other manuals are frequently updated pro moting committment to their use. - Safety meetings in all operating areas are always scheduled at the same time and frequency. The meetings are held whenever the crew is on the Monday evening shift. That means the "A" shift in each unit has their monthly safety meeting at the same time. Safety meetings are held on overtime by bringing the shift in early. There are no excuses or exceptions as it is considered "part of the job. " - Maintenance personnel including contractors attend weekly safety meetings held each Friday morning. - Permanent hose testing stations are in use which help in promoting their use. - Fire permit has space on it for extension so that it does not have to be rewritten each shift. - Each unit has halon fire extinguishers for putting out computer fires. - Safety Department conducts periodic checks on unit's safety checklists. /rrg SC 13370 LAM014591 STG 2430340
