Hammann There were no Recordable Injuries in April. Minor injuries have increased in May (10 so far in the month), so we must increase our awareness to maintain our safety record at the present high level. Areas having a high cumulative monthly rate are: Dept. 53 Insulators Truckers--Laborers Electricians Analysts Areas having a high cumulative monthly Injury rate, but showing improvement in recent months are: Painters Dept. 50 Areas having excellent safety records are: Dept. 513 -`8 months without an injury Stores - 15 months without an injury Dept. 33 - 16 months without an injury C. Contractor Safety--R.L. Hammond No report. II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC)--G. A. Danner A. Practice of Removing Manways from Equipment SAC recommended to PMSB that manways on equipment can be safely opened without blinding provided that: 1. The equipment is blocked, locked, and depressured. 2. The equipment is decontaminated (assured by vapor test for contaminants and adequate oxygen for breathing). 3. Manways are attended while open. It was noted that if a person inserts his head inside the manway it is considered vessel entry and would require an entry permit (P-0117). This practice was approved by PMSB with the following additions: a. The procedure applies to inspection only. b. Manways must be replaced immediately after the Inspection is completed and equipment must be inerted. c. This work requires a Special Permit. A SHAC Alert will be issued stating this policy. Removal of bolts from manways before blocking, locking, decon taminating , and blinding PMSB agreed with SAC's recommendation against this practice. III. REPORT OF THE PLANT SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC)-- A. H. Conrad A. Hourly Safety Committee There was 100% attendance at the April meeting at which Greg Daues discussed the plans for expanding S-T-O-P to all hourly employees. A status report was given on the following Items: 1. Cooling tower fan brakes - A design is being completed and this will be reported through normal maintenance channels. R.L. Hammond will check on this project and review it with Gene Tromblee as soon as the recommendation is prepared. 2. Vessel opening without blinding - SAC reviewing for plant policy (see SAC report). 3. Ose of barricade tape - Reminders have been issued to supervision and individuals were asked to report misuse of the tape to appropriate supervision. 4. Radio Calls - Problem explained to maintenance supervision. 5. Plectron checks - Plant testing of the plectrons in units will commence 5/16/83. New Concerns: 1. Responsibilities of holeman - SHAC will review and make a recom mendation to SAC and PMSB. 2. Other items discussed were referred back to the unit or craft where the problem originated. B. Quarterly Safety & Housekeeping Inspection This will be scheduled for the last two weeks in June. An Expanded Staff meeting will be held May 25 at 14:00 hours to discuss plans for the Second Quarterly Inspection and review the results and rating system from the First Quarterly Inspection. C. Annual Inspection It is assumed that this year's inspection will be similar to last year's with no formal outside team unless some reciprocal agreement is reached with Chocolate Bayou. More information on this is expected after a meeting between Tromblee and Brasfield on 5/20/83. LAM003671 Sc 02464 PMSB Meeting Minutes--May 18, 1983 Page 4 D. Drug and Alcohol Abuse Program SHAC reviewed a film acquired by Jay Cross, Chairman of TCSC. It was an excellent film, but was not suitable for our plant at this time. At present, the focus is on education. We need to be sure we have checked with other companies in the area who already have programs in existence. E. Bicycle Safety SHAC will work on this from a safety rules aspect only. Procurement, maintenance, and security of bicycles will be addressed by other disciplines. F. S&PP Survey 1. Nitrogen back-up of instrument air - A memo was written requesting that a change from instrument air to nitrogen be noted on the protective devices bypass status board in addition to the present control procedures. 2. Portable buildings and trailers - Those portable buildings in unacceptable locations have been removed. Several in questionable areas are not being moved as the risk is considered to be accept able . IV. REVIEW OF INCIDENTS Monsanto Incidents Date Incident No. Description 03/31/83 83-05 (not issued) Recordable Injury--Dept. 51 NH3 bullets; operator fell, same level, struck chin and neck required 3 stitches to close. (Fiesinger) 03/30/83 83-06 (not issued) First Aid Case--PA storage tanks; pipefitter and contract insulator struck by PA while working on 56P703. (Coleman) Contractor Incidents 04/07/83 C-83-02 (not issued) Days-Away-From-Work Case--Dept. 35 Dismantling; contract wrecking crew member; right abdominal strain; 4-7 days off. (Vornkahl) It is felt that investigation and reporting of incidents is taking too long. Steps should be taken to complete the investigation in a timely fashion so that effective corrective action can be taken. SC 002465 003672 PMSB Meeting Minutes--May 18, 1983 Page 5 V. STATUS OF S-T-O-P PROGRAM--G. W. Daues The card tally has picked up. This apparently reflects that goals have been established in this area. STOP III is in progress for the Chiefs/ Leadmen and instructors have been selected for the final two sessions for this group. The Office Safety Committee is at work on revising the program for laboratory and clerical personnel, and two members of the Hourly Safety Committee have been designated to work on the STOP program for hourly personnel. A new class for supervisors has been considered since there are approximately 10 people who have transferred to the plant from other Monsanto locations since the training was completed. It was suggested that the Hourly Safety Committee be polled for sugges tions on ways to motivate hourly personnel for improved participation in this program. VI. MISCELLANEOUS--R. T. Hammann A. Fire, Entry, and Special Permit Procedure--P-0117 The first session for reviewing the revised Fire, Entry, and Special Permit Procedure (P-0117) was held Tuesday, May 17. A second session for supervisory personnel will be held Thursday, May 19, at 13:00 in the Research Conference Room. These meetings are being held to ensure consistent implementation of this procedure throughout the plant. Plans are to implement the new procedure and permits on June 1, 1983. B. Eyebath/Safety Shower Retrofit It was a consensus feeling in the plant that the cost involved in retrofitting all safety showers and eyebaths for freeze protection and installing dual eyebaths is not justified. It will be left up to the units to determine whether or not it is necessary to retrofit any or all of the safety showers and eyebaths in their area. C. Emergency Procedure Manual This manual is out of date and needs a general revision. The following assignments for rewriting the manual were made: 1. Organization and Activation of Plan - Hammann 2. Plant Coordinator - Hammann 3. Personnel Section - Kaldenberg 4. Production Section - Ryan/Trafton 5. Maintenance Section - Hammond 6. Communication Section - Metten Bob Hammann will review with each superintendent what revisions need to be done in their section. The goal for completing these revisions is Sept. 1. At the June PMSB it will be determined whether or not this goal is realistic. After this manual has been updated an emergency drill will be held to test our preparedness. It is possible this drill may be held during the Annual Inspection. D. Distribution of Safety Binders Loose-leaf safety notebooks for supervisors and for holding the Safety Procedure Bulletins have been received and are presently being distributed. E. Quarterly Safety & Housekeeping Inspection Responses to the comments made in the Quarterly Safety & Housekeeping Inspection were slow in being received. Several phone calls were required in some cases in order to get the appropriate response. Since these responses are now being sent to the appropriate General Superintendent, in the future SHAC will not contact the individual, but will contact the General Superintendent concerning areas which are late in reporting. VII. PLANT MANAGER'S COMMENTS Recently we have seen an increase in the number of minor Injuries and incidents occurring in the plant. This could well be a sign of a let down in our safety awareness.
