Refinery Sept. 3, 1974 SUBJECT: August Safety Meetings During the Safety Meetings of August, 1974, the safety film entitled "Reaching for the Lightning" was shown. The regular monthly meetings by operations, maintenance and laboratory were well attended along with three special showings for office personnel. Safety suggestions and their status are as follows: Aug. 6, 1974 Shift No. 3 No safety.suggestions Instructor - Kucera Aug. 8, 1,974 Group I Instructor - Kucera 1) Charge pump at the BTX needs a mono-rail to lift out for repairs. Status This is a valid request and a work order has been issued. 2) No. 5 Compressor at old No. 2 Plat., the hydrogen line leaks. It cannot be cut off when compressor is being repaired. Status Work order issued. 3) Repair hole in front of Cat Cracker. Status Completed 4) Caustic Is being drained through air hoses. Status This was discussed at safety meetings and with supervisors. Aug. 13, 1974 Shift IV No safety suggestions Instructor - Kucera Aug. 15, 1974 Group II Instructor - Kucera 1) Sub-contractors were noticed violating Champlin's safety rules. Status All sub-contractors were notified and given a list of "Sub-Contractors Safety Rules" with emphasis on complying with the rules. 2) The permanent ladder on Heater 2A at the No. 3 Crude Unit was not secured at the top. Status After surveying the ladder, it was found to be not secured at the top, bent foot bottom of cage was not flared. A work order was issued to correct these sltuatioaspn i o ]974 AN 000019 SAEEDf. Status The ladder at the Cat Cracker was removed to the carpenter shop for repair. The ladder at the Vacuum Unit was found to be safe and remained at location. Aug. 20, 1974 Shift IV Instructor - Kucera 1) Light hydrocarbons are beening used for washing. Status This ugly duckling arises ever so often. All employees know that this is strictly against company policy. During the October safety meetings, a review of this violation and several others will be discussed. 2) The permanent ladder south of the Cat Cracker has an uneven spaced rung. Status In the true sense, this is a violation of OSHA Regulations. The true nature is not such that would warrant immediate correction. The safety .hazard involved is minimal. We suggest correcting this hazard during the Cat Cracker turnaround. 3) A platform is needed over the gas ballast drum at the No. 4 Plat. Status- The area supervisor, 0. C. Howard, and I checked on this suggestion. We found a true safety hazard and the situation is being corrected. 4) The wooden steps over the firewalls at Tks. 1001-2, 1014 and 2000 need repairing. Status A work order has been written to correct this situation. 5) A large hole near this pump at Tk. 2005 should be filled. Status The area supervisor was notified about correcting this problem. Aug. 22, 1974 Group III Instructor - Kucera 1) Wooden steps at the 1001-2 need repair. Status Work order issued 2) A fiberglass basket to be used by the electricians on the cherry picker is needed. Aug. 27, 1974 Shift II Instructor - Kucera 1) The grating above the sewer hole between the #1 - 2 Crude units is borken. Status In the process of being corrected.
