MINUTES OF THE SAFETY AND HOUSEKEEPING ACTION COMMITTEE December 29, 1976 Present: Jim Cavender Arnold Glass Harold Lacy Gene Stacy John Tissue RELIEF VALVE PROGRAM In the report on the collapse of the waste alcohol storage tank (954-T30), J. B. Cook recommended that four actions be undertaken by SHAC. The recommendations and SHAC actions follow: Recommendation 1: A comprehensive study needs to be initiated to develop a procedure to insure safe and correct installation of pressure/vacuum relief devices on equipment. Recommendation 2: An assignment with high priority should be made to review all conservation vent valve installations for adequacy of design. SHAC Actions 1 & 2: A specific request would be made to Plant Engineering Services Department for a recommendation for procedures to insure: (1) safe and correct installation of pressure/vacuum relief devices on equipment throughout the plant, and (2) proper continuing testing maintenance and operation of these devices. J. A. Glass will be the SHAC contact for this study and recommendations. Recommendation 3: A SHAC action request should be issued to all operating departments calling for the verification of the accuracy of the relief valve catalog specifications versus what they actually have installed. SHAC Action 3: The SHAC action request will be issued as soon as the Plant Engineering Services Department assignment is made. Recommendation 4: This incident should be widely publicized to all operating departments because of the frequency of occurrence of this type of incident (collapsing tanks by vacuum) and the likelihood of its happening again. SHAC Action 4: A brief description of the incident of December 11 with picutres of the tank will be placed on all bulletin boards to call attention to the damage that can be done when a tank is subjected to vacuum. SC 13626 LAM014845 SHAC Minutes - 12/29/76 Page 2 1976 INSPECTION REPORT RECOMMENDATIONS Assignments have been made and work is in progress in developing answers to each of the items listed. P-BULLETINS STATUS All of the Safety P-Bulletins that were being printed have been distributed throughout the plant. It should be pointed out that 13 Safety P-Bulletins have been revised and updated and published during 1976. This is almost half the total 28 Safety P-Bulletins in the P-0100 series. The remaining bulletins will be scheduled for review during 1977. GAS SPILL PROTECTION A PAFE is being prepared and the justification is being written for gas spill protection for the styrene plant. The project will provide a high air intake a blower, an absorption bed and scrubber, and ductwork to the control house and office building. An Engineering Work Order has been issued for a parallel study of gas spill protection needed for areas in the West Plant as well. "UNIQUE HAZARDS" The term "unique hazards" refers to those hazardous or potential hazardous conditions that exist in units or operating areas for long periods of time that tend to become regarded as "safe." One unthinking person could come in contact with such hazards in an unusual way that could result in injury. These kinds of potential hazards may exist in all units throughout the plant and require a renewed sense of awareness to them in order to get them corrected. Each supervisor is urged to survey his/her own areas for such hazards. OTHER ITEMS The committee reviewed a number of items for future action: A. Task Force Acknowledgement A number of persons have spent a great deal of time and effort in serving on Task Forces in preparing detailed analyses of complex situations in order to develop new and improved procedures. SHAC is preparing a recommen dation for proper recognition of the work of these task force members. B. Plant Traffic It appears that the plant traffic situation is getting worse again and will require some planned program for further reduction of exposures. This will be reviewed with Shorty Albritton and appropriate actions will be developed after that review. SC 13627 LAM014846 SHAC Minutes - 12/29/76 Page 3 Other Items (cont'd) C. Plant Fire Alarms Harold Lacy reported that he could not hear the plant fire alarm on the second floor of the main office building. He was reminded that each department was responsible for making necessary corrections to fire alarm system, so the proper notice could be given to people throughout the plant. D. Defensive Driving Course The notice went out throughout the plant. It was put on DIAL-5 on December 29. Enrollment is already underway. E. Blocking of Dedicated Walkways Gene Stacy pointed out one of the most frequent blocking of walkways was by bicycles improperly parked in walkways and sidewalks throughout the plant. It appears that an overall plant program is needed to correct the situation. F. Train/Truck Collision Gene Stacy summarized the incident report on the train/truck collision that occurred on December 19. He recommended that flashing lights be installed at railroad crossings to be activated by moving trains in the plant. Safety meetings throughout the plant should deal with rail road safety during the first quarter of '77. G. Cover Goggle Requirement for Users of Contact Lenses The PT Safety Committee has recommended the adoption of special cover goggles for persons wearing contact lenses. These would serve to notify other people that the wearer has contact lenses so that first aid could be properly administered in case of emergencies. Stacy stated that the whole policy regarding the requirement of the use of cover goggles for wearers of contact lenses should be reviewed. It was his position that Plano safety glasses are adequate protection. The safety department (Glass) will contact S&PP and National Safety Council regarding the latest up-to-date information on proper eye protection for wearers of contact lenses. SC 13628 vf J. A.
