Bourget I am concerned that several potentially serious - safety incidents have happened in 84 Bldg, in the last few weeks. Many of these incidents appear to have been caused by inattention to details, which is a bad habit for any of us to get into. Some examples are: 1. Valves have been found open on the kettle bottom lines leading to the blowdown tank, the seed strainer, and on the blowdown tank itself. In one case this caused a discharge of VCM into the work room. 2. The valves in the virgin VCM system have been found arranged so that VCM was being withdrawn from one tank and returned to the other. 3. Lines containing VCM under pressure have been opened while maintenance operations were being performed farther down the line, causing a discharge of VCM vapor into the work room. 4. Batches have been allowed to go out of control without taking proper corrective action, in two cases lifting a relief valve. This is almost unheard of in the Paste process, and can nearly always be prevented. 5. A fork truck tipped over after it was inadvertently run up on the curb. This one was classified a near miss and is being investigated. These are some of the potentially serious incidents which have occurred recently. No one has been hurt - yet. . However, with the exception of the fork truck accident, every one of these incidents could have been prevented by paying attention to the safety details of. your job, following standard procedures or at leasts asking questions of your Foreman when in doubt. I realize that many of you are relatively inexperienced in Paste operations, but that is just all the more reason for you to stop, think carefully, check the standard procedure, or ask a question before proceeding with an operation in which you are unsure. And, of course, there is no excuse for inattention to safety details under any circum stances. You are responsible for your own safety, and when you work with others, you must also be concerned with them. Please resolve now not to allow yourself to become involved in any more situations like the ones listed above.
