WADDELL, JR., M.D. SUBJECT: PULMONARY REGISTRY REVIEW, EMPLOYEE This case is reviewed because of a possible pleural plaque with calcifi cation noted on the chest X-ray. This employee joined Shell's Deer Park Manufacturing Complex as a general helper in 1955. By 1967 he was classified as an insulator's helper. Later job classifications included insulator #2 and insulator #1, pumper gager. By 1967 he had become an operator in the lube A section. The chest X-ray from 1987 showed a thickened pleural area in the right upper lateral chest wall with a linear calcification present. A film from 1984 examined with the advantage of already knowing the subsequently developed radiographic picture in that zone shows what could be some haziness consistent with pleural thickening. Oblique views were taken in 1984, however, and the X-ray reports "these views fail to convincingly confirm.the presence the of a previously suspected non-calcified plaque in the'" upper right thorax." The X-ray report on the corresponding PA view had indicated that there could be a non-calci fied plaque in that area. Also a chest X-ray with obliques taken on 09/05/86 was interpreted by the radiologist as showing "no calcified or non-calcified pleural plaques are identified." However, the 1987 film reading by Dr. Patrick M. Conoley, a NIOSH certi fied B-reader radiologist indicated that there was probably a calcified pleural plaque in the right upper lung zone. Considering job exposure history and his chest X-ray find ings, it is my professional opinion that this case should be considered a calcified pleural plaque, for purposes of counseling the employee (if he has not been already) and for purposes of OSHA 200 log recording. Corre spondence in the chart from Dr. Kern would indicate that it is reasonable to believe that the diagnosis was established in 1987.
