Changes resulting from this re include: 1) The BFG Safety Department issues all i hazardous area confined space entry permits, 2) The contract worker must have a BFG employee in attendai during confined space entry and 3) Require that the isolation points of all lines, pipes and connectioncapable of introducing flammable, toxic or corrosivt agents into the confined space be identified with ar equipment isolation tag and that each isolation poir verified by the permit issuer. Plant-wide distribution and training on the plant's revised `Hot Work' and 'Vessel Entry4 permitting requirements was completed on November 16, 1989. The BFGoodrich 'Confined Space* entry procedure has revised to include the requirement for continuous monitoring for flammable or explosive agents and oxy - content.
