Reports The Monthly Safety and Housekeeping Audits and Injury Summary for December were reviewed: Several items concerning vibrating lines were noted. Some of these were at lower than normal production rates and corrective work is underway. It was felt that the unlabeled switch gear in the DDDA ECR, though not turned over by construction, should have been locked out by operations as it was located in an active bank. DDDA was complimented on a very good job of danger tagging. Improvement could be made with better coordination between production and mechanical as to the "reason" for tagging. The outlying area received good comments for the overall good appearance and the posting of procedures at the Acrolein stations. Several minor type items were noted which indicates we need to stay on our toes to maintain our standards. S F was charged with developing a standard for the upkeep of plant-owned breathing air cylinders -and the periodic change out of air._ ; Congratulations to. 570 Lab, Machine Shop, "J" Unit Reaction, Op. 1 and 518, 519 8 509 T/F as Good Housekeeping Award winners. The two unsatisfactory areas have been returned to acceptable standards. II. Incident Reports j Reports 78-75 and 78-76 were reviewed: 78-76 - The use of gloves at this rotating equipment has been re-evaluated with the conclusion that gloves in this situation would present additional hazards. The policy remains that gloves are required except where exempted. Ill. Safety Activities Committee - H. F, Charlton On Plant No lost or restricted workday cases. Four CPR Training classes are scheduled for January and February. To date, 150 people have: received CPR instruction. Pickup Truck Passengers - H. F. Charlton After investigation and discussion, inducting other plant sites, the Committee recommended that the present policy of allowing passengers in the back of pickup trucks be continued. VI. Process Hazards Management - M. A. Ervin Mike reviewed a Safety and Fire Protection Reference Guide draft concerning Process Hazards Management. The guide deals with the application of management controls to process hazards through programs, procedures, audits, and evaluation. Process Hazards Reviews are just one na!rt of the overall Process Hazards Management. ! Began with the 1965 Louisville explosion. 2. First corporate guidelines issued in 1968. 3. Bulletin 505 issued in 1973. 4. Process Hazards Management Guides (to be issued in 1979). The Victoria Plant Process Hazards Management Committee structure is not complete, but a condition for not having a Department Coordinator is that all management reviews cover Process Hazards. VII. Respirator Fit Test - J. M. Kayser j The facility is comoLete and has been satisfactorily checked out. Respirator repair and cleaning can begin immediately - see I/H Procedure No. 7. A memo will be issued outlining the start of the fit test program. The attached chart shows the fit testing requirements. VIII. Plant Ambulance Relocation - M. C. Dorris : A relocation recommendation will be worked: through the Emergency Preparedness and SiterDevelopment Committees. IX. Grass Fire Control Capabilities - J. V. Woodrick Recommendations will be made by a committee composed of H. N. Harris, V. C. Branch and J. E. Borden. X. Key Safety Data Significant improvements were made in the areas of high potential incidents. We also have a good string of injury free days on plant.
