Gros, Lead Tankerman involved didn't reveal very much more than he had indicted in his original statement. Additional items were he had not added stabilizer in about a year and a half; he dealt with two different operators (addition started with one and completion occurred with another). The primary cause was his failure to leave a valve open on the pot to take care of the expansion. Some of the VC operators handle the stabilizer addition with the tankerman standing by and some add the stabilizer addition themselves. Discussion with Clark Graybill led us to agree that the VC operator should handle the actual stabilizer addition and clearing of the pot. Our new loading procedures that will soon be typed up had reflected this setup. We will implement these changes as soon as the written * procedures are available. JCL/syr 102215 SI;
