Comments on Statistics - R. T. Hammann There were no recordable injuries in May and there are no recordable injuries so far in June. We still need to keep awareness high due to the two major shutdowns occurring in August and October. SC 13796 STG 2430602 LAMO15545 PMSB Meeting Minutes - June 27, 1984 I Page 2 Paul Bailey reported that a 12-rainute safety tape has been pro duced for the AN-5 shutdown and is currently being edited by Joe Swain. This will be used for all Monsanto and contract personnel involved in the AN shutdown. In addition, 5-minute safely meetings will be conducted for both operating and maintenance personnel at the beginning of each shift to address work to be done on the shift (potential hazards, special precautions, etc.), equipment to be used and any problems encountered on the previous shifts. Areas having a high or increasing cumulative monthly injury rate are: Dept. 48/56 Dept. 33 (most of problem due to air contamination incident) Pipefitters Pumper/Gaugers Power 2 Instrument Dept. Areas showing good or improving injury rates are: Dept. 44/45/46 Boilermakers Stores Dept. Dept. 50 Carpenters Areas having excellent safety records are: Painters HCN Complex Insulators Dept. 513 - 6 months without an injury - 6 months without an injury - 10 months without an injury - 21 months without an injury II. NON-MONSANTO SAFETY - Jeff Peterson A. Contractor Safety There were no recordable injuries in May. However, there was a lost workday injury in June when an employee of Continental Slip Form Builders suffered a bruised left foot when hit by a section of drilling tower while dismantling it. B. CEP Safety There was very little activity and no injuries in May. III. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC) - G. T. Ryan A. SAC's evaluation of the recommendations from the SHAC critique of the Emergency Procedures' Drill was reviewed. Two recommendations were returned for additional review to SHAC. 1. A task force will be formed to review the telephone communications in the Control Center. 2. Development of an improved system for better dissemination of information on the alarm code system and phone numbers for the emergency control center (i.e. information on wall plaques at instruction points). SAC will also plan a walk-through on Friday, June 29th to ensure proper functioning of the streamlined control center operation. IV. REPORT OF THE PLANT SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) E. P. Bailey A. Hourly Safety Committee 1. Feel there is still a problem with vehicles parked outside designated parking areas in the plant without keys in the ignition. SHAC will work on devising a plan for improving this situation. 2. Mew members on the Hourly Safety Committee are: Madeline Hair, Robert Kozlek, Daniel Nelson and Ben Peek. B. 2nd Quarter Safety and Housekeeping Inspection Winners of the surprise inspection were Dept. 50, CED Warehouse, Machinists and Control Lab. Discussion followed on plant acceptance of surprise inspections. It was decided that we will alternate between surprise and scheduled inspections in the future. Responses to the inspection reports are due to Steve Moncla by July 15, 1984. C. Contractor Safety The Contractor Safety Procedure has been reviewed by the Legal Department and approved by Gene Tromblee. It will be typed in procedure form and issued. D. (Supak) VI. MISCELLANEOUS - R. T. Hammann A. General Permit Training has been completed, the procedure has been issued and personnel training in the units is in progress. Implementation of this procedure will start July 2. B. Barricade Tape New barricade tape with captions inscribed on it is now available from stores. The yellow tape used to designate caution or danger has the wording "Caution - Do Not Enter" on it in black letters. The red tape used for chemical spills or fires has "Entry Requires Special Permission" on it in black letters. Information on this tape will be disseminated to plant personnel. C. Video Tapes Information was handed out concerning new video tapes available from the Safety library. VII. PLANT MANAGER1S COMMENTS - G. L. Tromblee We have had an excellent safety record so far this year. Our recordable injury rate is the lowest among MFI plants. We need to continue this good effort through the upcoming major shutdowns. Frank Reese will be here next Tuesday and will make short visits to units in the plant with a walk-through of AN-5. He will speak to expanded staff at 2:30. 13799 /dgb R. T.
