Tullis Date September 20, 1981 Subject MONTHLY SAFETY MEETING - OPERATIONS On September 15, 1981 "C" Shift held its monthly Safety meeting. Those in attendance were: Randy Gantz, Richard Dungan, Fred Graham, Lee Steward, Mike Roe, Chris Turner, John Olson and myself. The topic of this meeting was VCM Tank Car leakage and bad hoses. Some of the items talked about were: 1. Weakest parts of the VCM cars, bullets and sphere. 2. The first thing to do in case of a leak is to insure personal safety and stop the source of the leak. 3. All personnel should know the correct valves to block in. 4. In the event a leak should occur, all that you would be able to do is isolate the source and let the fire burn itself out. Also, keep nearby equipment as cool as possible. 5. Water also helps dissipate VCM. 6. Remember to always stay upwind of the fire. 7. If a compressor should trip out in the tank farm, it should be checked out and find the problem, i.e., high pressure on the car due to popped check valves or low oil pressure on the Corkin. 8. Install pressure indicators on the panel for the railcar unloading platforms. We also discussed the incident with the failure at the VCM liquid hose and the employees did use the correct procedure at that incident. Randy Gantz then discussed the accidents and near misses since the last meeting.
