Approximately 50 percent of member companies did not report safety statistics for the first nine months of 1983. Since the data are cumulative, those companies who have not submitted quarterly data but who do sumbit year-end data will still be eligible to win the Lammot du Pont Safety Award. I am attaching a copy of the reporting form for your use. I am also enclosing revised rules for winning the Safety Award and the Certificates of Honor and Achievement. Finally, I am enclosing an address update form for you to complete. Please return this form indicating the name of the person in your company who should receive the Occupational Illness and Injury Report and all related correspondence. Also, please indicate the number of copies of the OIIR that you wish to receive. CMA continues to urge your support in the safety statistics program. As part of this promotion, CMA will recognize significant safety performance at the annual meeting of the Association. Three plaques, one in each group described below, will be awarded for the greatest improvement in safety performance by member organizations. The plaques are known as the Lammot du Pont Safety Awards in honor of his outstanding contributions to safety in chemical manufacturing. A Certificate of Meritorious Safety Performance will be awarded to the "runner-up" in each group. A. Eligibility Any company who has been a member of CMA for at least one calendar year is eligible if it has reported its annual injury experience to CMA for the previous five consecutive years prior to the award. The injury data are based on Occupational Safety and Health Administration reporting requirements. The award is given for improvement in safety performance, calculated by CMA for each company as outlined below (Section C). The annual Total Recordables Incidence Rate experience of a member company shall be the total occupational injury and illness experience of the organizations's operating units. Any division of a company paying an individual membership, however, and reporting for the appropriate periods, will be eligible for the Award. B. Number of Awards - Three Member companies reporting injury experience are to be divided into three groups, namely: Group 1 - Member companies having total annual employee exposure hours* during the award year of 2,000,000 or less. Group 2 - Member companies having total annual employee exposure hours during the award year of 2,000,000 to 20,000,000. BOR 005428 -3- E. Type of Award and Presentation The awards, three bronzed plaques, will be presented by the Association's Chairman of the Board of Directors or the President at its next annual meeting. The three organizations winning Certificates of Meritorious Safety Performance will be recognized at the annual meeting. It is recommended that the recipients give suitable notice to all of their plants or operations participating in their safety record. Revised January 9, 1984 RPH:cd BOR 005429 CHEMICAL MANUFACTURERS ASSOCIATION CERTIFICATE OF HONOR CERTIFICATE OF ACHIEVEMENT A. Eligibility Any individual plant and/or. laboratory of a member company which regularly reports its quarterly injury experience to the Chemical Manufacturers Association and which works a minimum of 20,000 employee hours during the year is eligible. Units such as home office, sales office, or other units not associated with a plant or laboratory are not considered eligible. NOTE: Plant or laboratory is defined as a self-contained unit of a company, not a subdivision of a larger plant or a laboratory. Self-contained means that a plant should manufacture one or more products under separate management control. A laboratory located at a plant site but whose management reports to management at another location could be reported separately from the plant. Example 1, A plant may be organized into production, maintenance, power, technical, accounting, and other groups, with supervision reporting to the manager of the plant. Only the entire plant is eligible for a Certificate since all of the units are parts or divisions of the plant. Example 2, A research laboratory is on the site of the plant in Example 1. It uses common utility facilities of the plant but the nature of its work is research and the laboratory management does not report to the plant manager. Since its operations are self-contained and separate from those of the plant, it is eligible for a Certificate. B. Classification of Injuries Certificates of Honor and Certificates of Achievement will be based on the Occupational Safety and Health Admininstration (OSHA) requirements for reporting occupational injuries and illnesses. C . Basis of Award To qualify for a Certificate of Honor the individual plant or laboratory must complete the calendar year without a death or lost workday case, as defined by OSHA, involving days away from work or days of restricted activity.
