Texas Chemical Council Award The most cherished safety award in Texas was! presented to W. T. Dodson, Safety Activities Committee Chairman, by A. D. Cyphers. Signs have been placed over entranfce gates stating "You are entering the safest chemical plant in Texas". II. The Monthly Safety Housekeeping Audits and;Injury Summary for April were reviewed. Safety shower eye wash stations that d not meet plant standards continue to be found. Areas are awaiting the final standard which is being revised before mak ing further revisions. ' I People not performing safety checks pep schedule is of major concern. A new relief valve has been received tb replace the inadequate one on the I-R analyzer house in 514. Labeling of the door at the base of the boiler stack, similar to Acids Power Area, is suggested. SOC markings on gages continue to be found illegible and missing. In several instances operation outside the SOC markings were noted. Relief valve mating flange identification was lacking at several installations. 1 There were four cases of potential fallling objects noted above first floor level. Many items listed should have been corrected by area personnel and not found during an audit'. There is a need to be more alert for the proper disposal `of aerosols and cans that contained flammables. i The relief holes in a safety blow air nozzle discharge pipe that were sealed off with electrician's tape is a safety and OSHA violation. The Mechanical Superintendents were asked to review eye protection requirements in shop areas arid to develop some consistant guidelines. ' The use of "Perclene" should be under rrgidly controlled conditions and a search for a replacement should con tinue. There were no High Potential iniuries occurring during April, 1979. III. Incident Reports Reports 79-14 through 79-19 were reviewed. The following addi tional comments were made: 79-15 - It was again emphasized that all relief valves with set pressures above 1000 psig should be scheduled for testing. 79-16 - Has plantwide significance and users of river water in confined spaces must remember to check with Nylon Power to see if Acrolein addition;is in progress, and to immediately discontinue use if an unusual odor is detected. 79-18 - The feeling of discomfort exists because the means of water entry into the catalyst has not been deter mined. 79-19 - John Chambard will revise S 6 F 48 to prohibit blanks under relief valves for any reason. Tom Thome will develop a blanking procedure. ; IV. Safety Activities - W. T. Dodson | On Plant - ! No lost work injuries - 522 safe days. Next Goal - 4MM safe hours about June 29. (Achieved only3 timespreviously) ! Safety rule cards for vendors rewritten. Production Newsletter on N02 dumps issued. Safety Notebooks to be distributed in May via safety contact. Third Lab "Test Your Knowledge" in progress. Technical bulletin on metallurgy issiied. CRAB distribution inprogress. ! DDC - 1160 employees graduated as of 5/1/79. Makeup assignments continue through 5/24/79. Sports mailout completed. Off Plant Safety training of new operators continuing (previous hires to complete by end of May). Laboratory On Plant Safety Program - L. M. Jackson The Laboratory On-Plant Safety Committee will present its program in two phases. "Phase I Hazarcious Chemicals" Phase II "Specific Training Program in'First Aid". PHASE I - This program will start with a series of films presented to all laboratory personnel. "25 Grams of Prevention" and Laboratory Safety. Every two weeks a set of six multiple choice ques tions "Test Your Knowledge" will; be placed on the safety bulletin board in each lajb. Questions will be general covering some concern's of Hazardous Chemicals. Four questions will be answered on back of sheet and two with no answers. At random a member of the On-Plant Safety Committee will ask for the correct answer to any of the "Test Your Knowledge" questions - if the correct answer is given that person could receive a Silver Dollar. I This phase of the program will end in approximately eight months. Each "Test Your Knowledge" question sheet will have a cartoon as an Eye Catcher: PHASE II -A specific training program which will include the use of the New manikin and defibrilator. This phase is now being set up by A. Eisman and plant doctor. The same "Test Your Knowledge" system will be used and will cover all phases of First Aid. The intent of this program (Phase I) is !(1) to keep the analyst aware of all possibilities when handling even the smallest amount of chemicals (2) be alert in what to do in case of spills, and (3) get acquaintejd with and use available materials and manuals that are! placed in all labs. Phase II of this program will overlap,! because of the number of analysts to receive this training, it will start as soon as possible. V. Industrial Hygiene Procedures 2, 11 and 12 were approved with some minor work changes to be made before being issued. VI. Vehicle Safety Check List - H. F. Charlton A check list to be placed in all plant vehicles was approved. VII. Indicators at Pipe Bridges - H. F. Charlton An employee suggestion was approved to paint lines on the road underneath pipe bridges to aid crane operators in determining when the headache ball has passed under the bridge. VIII. Stainless Steel Bolting - D. G. Jentsch j Approval was given to remove all B8 Class I bolts from stock and replace with B8 Class_II. Future stores purchases will be B8 Class II. IX. Relief Device Plugging Survey - J. L. Chambard The survey results are attached for information. Response to the recommendations will-be made at the June Central Safety Meeting. X. Key Safety Data High potential incidents, incident frequency r'ate and off-thejob injuries have increased at an alarming rate. RECOMMENDATIONS 1) Consider RD close to source to be relieved in services subject to plugging. 2) Confirm chemistry -- use information to:design systems. 3) Increase check frequency to 3 mo. where I plugging is a problem -- adjust based on experience until source is characterized and problem corrected. 4) Develop procedures for checking inlet lines. 5) Make sure procedures do not allow blanks under relief devices. !
