Francke Reactor Building - Drums not labeled - 1st floor. - PVC slurry flowing across walkway in front of washout tank area. - Letters missing from "Restricted Area" sign in front of building (facing roadway). - Third Floor - Flange at relief line inadequately bolted. Some nuts were holding by only a few threads (#7 Rx). - Gauge on relief system (between rupture disc and relief valve) missing needle (#2 Rx). - A "Restricted Area" sign should be posted at the entrance from the newly installed walkway from the construction area. - AMS spilled on floor under drum spout; drum not grounded (drip pan needs to be provided). - Gas masks should be checked for compliance with OSHA regs and (they should be cleaned periodically). - Scott air packs should be wall-mounted to facilitate donning (some are, some are not). - Stripper area - raised floor grating creating a tripping hazard. - Large valve laying on second floor landing of outside stairs (south side). Finishing Building - Broken light bulb overhead near east entrance. - Unguarded light bulbs (5) above walkway over rotary dryers . - Screener level - Broken wooden ladder laying on floor at north wall (not tagged out). - Fire extinguisher blocked by rope used to hold screens against wall. - Angle iron used to repair south screener projects into normal passageway (should be covered or shortened). - Broken plexiglass in door leading to outside platform. Primeau -2- 22 January 1976 - Screener Level (Con't) Belt drives for blowers not completely enclosed allowing easy access by personnel, Portable heater in bagging area office had an electrical cord with no strain relief and taped repairs. Warehouse Area - No label as to contents on a bottle of some chlorinated hydrocarbon solvent located at stamp cabinet. - Straight ladder in warehouse next to stamp cabinet did not have safety feet. - Stamped warning (PVC contains.VC, etc) on bags of PVC resin illegible (5 instanced observed). - Broken pallets and empty cardboard boxes were piled against the west wall creating a fire hazard. Other Areas - Barricade tape separating non-regulated construction area from regulated area down in a number of places. - Motor control center between finishing and reactor buildings had a number of doors open exposing live wires. Both high and low voltage cabinets were involved. General - Weekly safety equipment checks should be followed up to make sure they are being done properly. - Items such as hoses and unused equipment and tools need to be controlled better^such that they are put away and properly stored when not in use.
