SEWAREN PLANT - MANAGER MODESTO AGRICULTURAL RESEARCH LABORATORYDIRECTOR This is further to our memorandum of this date on the Chemical Products Toil city Manual in which we made reference to a physical examination program for employees handling or exposed to CBP-55 end OS-1199* The purpose of such a pro gram is to provide maximum protective measures for the employees exposed. It should also serve as a cbech on our safety standards and practices. The proced ure, which is to he put into effect immediately, is as follows: 1. An initial examination prior to the time the employee commences work with CBP-55 or OS-1199, or In the case of employees presently working with the products as soon as arrangements can he made td put the procedure into effect. 2. A re -eramination three months after the Initial examination, pro viding there is any significant contact during that time. 3. Further re-examinations at six-month intervals thereafter, if there is continued significant contact during that time. 4. A re-examination at the time he ceases handling the products or is assigned to other work, unless he has been exposed for less than one month in which event re-examination should not he necessary. 5* A urinalysis at two-month Intervals during the entire period of exposure. The following forms will he used in connection with the examination procedure. 1. Form I - Occupational History This form is to he completed before the employee is sent to the physician for the initial examination and a copy given to the examining physician for his information. 2. Form II - Physician's Report of Examination This form is similar to our standard physical examination report Form SR-lSOO hut has been expanded to include additional information believed necessary for the purpose of this program. ABS-044351 2 3. Form HI - Special Re-examination This form is to be used to record the results of the periodic re-examinations. It may also be used in the event a sudden acute exposure or suspected chronic exposure necessitates referral to the physician. 4. Form IVA - Complete Blood Count This form is to be used in conjunction with Forms II and III. 5. Form IVB - Urinalysis This form is to be used in conjunction vith Forms II and III and also to record the bi-monthly urinalyses. The forms were prepared by Dr. C. H. Hina, Consulting Toxicologist.for the Shell Companies, who will review and evaluate the reports and make any nec essary recommendations. They should be prepared in triplicate, and the original of each form sent to Dr. C. H. Bine, c/o Dr. K. R. Edlund, Shell Development Company, Emeryville, California. One copy should be kept In your files in the earns manner aa other physical examination reports, and the other may be retained by the doctor for his future reference. A supply of the forms Is enclosed. Dr. Hine has pointed out that because these examinations are a new venture they may cause a certain amount of apprehension on the part of employees, and that the apprehension might tend to cause some individuals to exaggerate minor disturbances or symptoms which have no relation to exposure to the chemical compounds. We feel, therefore, that it would be advisable to discuss the program personally with the physician who Is to make the examinations, commenting upon that possibility and asking him to make the examination in as objective a manner as possible so that a statistical compilation of possible complications may be made. In that connection, it has been found from toxicological studies that the chemicals CBP-55 and OS-1199 are Irritating to the skin, eyes and respiratory tract. There is no evidence from animal work or from careful examination of our employees to date to indicate, that they produce damage to the liver and kidney or other serious effects. However, because of their atruoture there are theoretical reasons to believe that liver and kidney injury might occur In man following un due exposures. You may wish to pass this information on to the doctor. Generally, proper safety practices should tend to avoid any undue dif ficulties arising out of handling of these compounds. However, Dr. Hine hew emphasized that In selecting employees, no person who has a history of tubercu losis or chronic respiratory disease, liver disease, kidney disease, or chronlo skin conditions should be subjected to repeated contact with these agents nor should persons who are alcoholics be permitted to work with either compound. Any questionable cases should be referred to Dr. Hine for review and advice before a decision is reached or the employee assigned to handling the compounds.
