This key indicator is warning us that if we do not change we are headed f c>r a more serious disabling injury. A program aimed at turning the trend around was presented by Arnold Glass. It was based on a successful program conducted at Springfield earlier this year. SAFETY CONTACT '76 was reviewed by the Board and was approved for a plant-wide program. PLANT MANAGER'S COMMENTS The serious injury frequency results indicate that we as a team are not performing in an excellent way. We must ask ourselves whether or not we are putting safety into the background, behind all of our other daily activities, instead of its being a major consideration in how we conduct our business. At the current serious injury frequency it will be quite difficult, if not impossible, for us to reach our 10 million man-hours with a major injury. Phil stated that he was not satisfied with the investigation and report ing of near-miss incidents, for example, in the future: 1. He expects to personally review the results of serious near-miss incidents within three (3) working days of the event.and receive a full written report within five (5) days. 2. He expects the General Superintendents or alternates to inform him on off-hours or weekends when serious or high-potential near-miss incidents occur. 3. He will confer with the dfepartment heads following initial notice of serious and high-potential near-miss incidents to appoint special investigating team to determine the cause of the incident and recommend appropriate corrective action. Experts and persons outside the affected department will be appointed to serve on these teams. SC 002569 STG 2401890 LAM003776 PMSB Minutes - August 17, 1976 Page 2 4. He expects the Loss Prevention and Safety Department to review and report on the status of the recommendations made by investi gating teams. The Safety Advisory Committee will review the P-Bulletin on Incident Investigations in light of these comments and recommend changes to comply with this increased emphasis on investigation, reporting and follow-up. SAFETY ADVISORY COMMITTEE - McCullough 1. Biennial Loss Prevention Review Proposal re-submitted for review was approved and was recommended for approval by the PMSB. It will be put into P-Bulletin form and circulated for approval. 2. Project Loss Prevention Review A new procedure has been submitted to SAC for review. (The new P-Bulletin is being circulated for approval.) 3. Ex-Battery Limit Pipelines Tom Ryan has issued an interim procedure defining responsibility and ownership of new pipeline construction in pipe racks. This procedure is in lieu of SHAC responsibility for re-writing the P-Bulletin covering this area. SAC recommendation for adoption of this procedure was approved by the PMSB. A special TASK FORCE is preparing the revised bulletin under direction from SHAC. 4. Follow-up on Emergency Procedure Plan Review A special meeting was scheduled to assign responsibility for resolving the concerns developed during the EPP review. Assign ments will follow. (Copies of program prepared by LP&S were distributed later to all department heads for use in training.) 2. Task Force Reports a. Flammable Drum Storage -- Second draft ready for review by SAC. b. Cylinder Storage -- P-Bulletin out for approval. c. Life-support Systems -- Sept. 1-target date may slip. REVIEW OF SERIOUS INCIDENTS The following incidents were reviewed by the PMSB: a. Laceration of finger of machinist--McCullough b. Near-miss -- Pickup truck ran into fence -- McCullough c. Serious incident -- Zirconium impeller ignition in Shop -- McCullough/Trafton d. Contract Security Guard Injured -- McCullough FIRE PREVENTION WEEK -- Glass Preliminary proposal for a night demonstration of fire crew at training grounds was discussed and tabled pending investigation of several inquiries by the PMSB. NOTICE OF BOARD OF DIRECTORS AWARD Phil Brubaker announced that the Plant received notice about the Board of Directors' Award on July 14 for operating 900 days without a lost-time injury. This is the first time our plant has ever received this award. The flag will be presented by Virg Waggoner at a later date. 2. All members of the board were urged to survey the status of their own programs for increasing the awareness of their people to hazards and for responding safely to the hazards. 3. REPORT OF SAFETY ADVISORY COMMITTEE (SAC) -- McCullough A. Fire and Entry Procedures The existing Fire and Entry Procedure, P-0117, does not clearly speak to ownership of new construction outside battery limits. We recommend that SHAC be assigned to revise this bulletin to adequately cover same with special emphasis given to perimeter pipe racks. Hendrick has been attempting to complete the revision of P-0415, Vacuum Truck Services. The subject yet to be resolved is assignment of xesponsibility. Analysis of this responsibility breaks down into three logical areas. They are: 1. Administrative Responsibility a. Vacuum truck coordination and planning b. Cost control and reduction c. Vacuum truck equipment testing and maintenance d. Resolution of complaints e. Safety and orientation training f. Out of battery operations 2. Functional responsibility where the vacuum truck is operating within the battery limit for a customer and at his direction. 3. Dispatching or scheduling vacuum trucks. SAC makes the following recommendations: 1. Responsibility for dispatching continue to be handled by Zone 1 maintenance. 2. Functional responsibility be squarely defined to the user with re-emphasis through procedure. 3. Administrative responsibility be assigned to either a full time contractor supervisor or a Monsanto supervisor. C. Housekeeping Inspection SAC recommends that these areas be eliminated from the housekeeping inspection tour: 1. All construction areas 2. EMA 3. LDPE Dismantling 4. Dept. 24 4. STATUS OF PREVIOUS SAC ITEMS -- Glass A. Biennial Loss Prevention Review The first draft of the proposed revision of the Biennial Loss Prevention Review was returned by the SAC with comments for revision. Plans for Annual Housekeeping Inspection Plans are currently underway to begin preparations for the -October '76 housekeeping inspection. These plans were outlined for the PMSB. Additional notices about these plans will be sent out as a SHAC Action Request. 6. REPORTING OF PROPERTY LOSS INCIDENTS - Glass Arnold Glass reviewed the purpose of P-0111 Reporting of Property Loss Incidents The purpose of reporting property loss incidents is to provide the Loss Prevention and Safety Section with the following: * 1. Property loss data for documenting performance. 2. Statistical data for the probability of loss events, for use in evaluating plant property risks. 3. Loss data for insurance claims and records. 4. Information pertinent to the Plant Risk Ausit a. Incidents involving items on the Audit Report list. b. Incidents illuminating items which possibly should be added. Property Loss Incidents shall be reported when they have the capability to result in property losses exceeding $500 (combined direct damage and business interruption); and/or in personnel injury(ies) of'serious or greater severity. Actual loss or injury is not a condition for reporting. Incidents shall be reported to the Loss Prevention and Safety Section within 5 days, using the "Report of Property Loss Incident" form. Forms have been given general distribution and are available from the Loss Prevention and Safety Section on request. 8. NITROGEN HOSE CONNECTIONS -- Brubaker Phil expressed concern over status of the nitrogen fitting problem and asked for a review at the next meeting of PMSB. He expects members of his staff to review their personal programs with him right away if they had not already done so. Phil commented on the memo to be sent to all plant supervisors regarding compliance with the plant fire permit procedure. The reason for the memo was the fact that many violations were listed in the guest housekeeping inspection reports. The intent of Phil's memo was to express his expectations for compliance with the approved procedure. SC 002577 STC 2401898 ^ LAM003784 PMSB MINUTES, 9/15/76 -2- October 16, 1976 SAFETY ADVISORY COMMITTEE (SAC) -- McCullough Mike reported that assignments of responsibility had been made for the various concerns developed during the recent Emergency Procedure Plan Review. Mike reviewed the activities of the SAC regarding problems caused by x-ray work in the plant. Stray radiation from gamma radiation sources used to x-ray construction welds was found to interfere with normal operation of level controllers and flame-out monitors ("Purple-peepers"). All x-ray work was banned until the nature, extent, and necessary corrective actions could be developed. A new x-ray control procedure will be proposed by Loss Prevention and Safety to protect the sensitive monitoring and control instrumentation from stray radiation. SAFETY AND HOUSEKEEPING ACTION COMMITTEE (SHAC) -- Lacy A. "Safety Contact '76" Program -- Glass At this point, all of the Topic Expert Cards had been distributed and about half had been signed and returned for assignment to Visiting Safety Contactors. Several members of the PMSB had made one or more contacts and were well pleased by their reception. B. Annual Inspection -- Lacy Progress in preparing for the annual inspection was reported by Harold Lacy. He requested comments on the proposed schedule and asked for detailed department plans for tours through the various units. C. Status Reports on Task Force Activities -- Lacy 1. Flammable Liquid Storage -- Proposed P-Bulletin is circulating for comments. 2. Protection from Spills -- This is a formidable problem - the task force is actively working on it, but much work yet to be done. 3. Ex-Battery Limit Work in Piperacks -- Task Force expects to have a proposed P-Bulletin circulating for approval prior to annual inspection. D. Electrical Wiring A SHAC Action Request was sent out asking for detailed inspection of all departments for loose wires on open-ended wires and identification of all electrical circuits at switches and breaker panels. Responses to this request were expected to be received by September 30. E. Nitrogen Connectors A proposed procedure for use of'Thor fittings on nitrogen hoses and hose stations was submitted and approved as an addition to the P-Bulletin P-0126 Nitrogen Distribution. Electrical Shock to Operator - Near-Miss - Rasmussen FIREFIGHTING DEMONSTRATION -- Glass The PMSB approved the proposal to hold a night demonstration of firefighting by the Fire Crew at the training grounds during Fire Prevention Week. (This has now been postponed because of west plant shutdown and housekeeping inspection.)
