The KOSHA inspection team was in the plant 16 days. The inspection was primarily on health problems. The following items were presented as possible citations: ABATEMENT PERIOD 1. 1910.134: The air line masks assigned to the OVA men were not properly stored. (Already Corrected) Immediate 2. 1910.132: The protective suit used with ATSC needs to be sanitized after each employee use. 5 Days 3. 1910.94: The sandblasters air hood needs to be sanitized after each employee use. 2 Days 4. 1910.212: The oscillating fans in Building 108 and the Carpenter's Shop were not guarded with 1/2 inch mesh. (Already Corrected) (This might be a repeat violation.) Immediate 5. 1910.23: Maintenance work platform on roof of 10 Days mill room and platform on Building 125 latex stripping column did not have proper guard rails. 6. Three locations were mentioned as needing eye bath and safety shower, as chemicals are dis pensed from drums. Building 2 second floor outside hot room in Compounding and Boiler House 7. 1910.141(H): Vending machines in lunch rooms of Compounding and Building 156 are dirty and need routine cleaning. .141(C): Trash receptacles need to be cleaned. .141(A): Floor and walls need to be cleaned. 5 Days 5 Days 5 Days NGC 14081 2- - .8 1910.141(C) : The women's change room In Building 126 did not have a covered receptacle. (Already Corrected) ABATEMENT PERIOD Immediate 9. 1910.141(A): The second floor of Building 121 had three severe process water leaks. 5 Days 10. 1910.28: Stepladder used to enter fluidizing bed was too short. (Already Corrected) Immediate 11. 1910.242: Three air hoses in the mill room with the 30 psi reducing nozzle cut off. 30 Days 12. Personal monitoring samples were collected and could be subject to citation, depending on the results. B/115 - VCM B/2 - VCM B/115 - Ammonia B/131 - Butadiene, Styrene, and VCN B/112-Nuisance Dust B/122 - Silica Dust Mill Room - Antimony Mill Room - Lead Mill Room - Arsenic (P-89) Other added comments: 1. They observed several situations where protective equipment was provided but not used as required. 2. They observed two men in Building 115 without respiratory equipment and the 5 ppm red light on. 3. They stated that employees do not believe our low personnel monitoring results. Employees particularly question the void samples. 4. The 1 ppm, 5 ppm, and 100 ppm lights/alarm cannot be seen or heard in Building 121 drumming area. 5. The pallet plant was the best they had ever inspected with regard to noise, guarding, and housekeeping. 6. They commented that the previous noise citation in the mill room could cause high noise levels throughout the plant to- be a repeat citation.
