Parker Subject: Plant Manager's Safety Inspection Departments 4-5-7-8-6-16 December 23, 1959 All Monsanto personnel working in each area were assembled in the control room of that area. Each group was told of the theme of the i960 safety program - I AM MY BROTHER'S KEEPER The need for extra vigilance to counteract the apparent let-down, after a long major accident-free period has ended, was stressed. Comments specific to particular areas developed as Indicated below. Department 4-8 These personnel were congratulated for continued overall fine housekeeping. Supervision were complimented on the orderly appearance of their office. The current report of inspection of safety equipment by depart ment supervision was checked. The south tank farm was inspected. The steam hoses used in con junction with educting water from the pits look unsightly. Educting is necessary since the drain sewer is almost completely plugged. A permanent drainage system has been thru various stages of design, and a PAPE estimate will be circulated soon. Weed Growth in these tank pits Is also In need of attention. In the T7 pit, a foundation removal Job was left uncompleted. The overall condition of the emergency generator room was good, but dust on 8EG1, and oil on the pad were noted. Also, the Job of installing a drain line thru a chipped section of the concrete pad was never properly completed. SC 16645 LAM023914 c3 2- - Department 5-7 These personnel were assembled prior to the Inspection of the facilities. Therefore, except for the chief operator and two department supervisors who made the inspection trip, the bulk of the personnel received indirectly the observation that the area showed good improvement crrer the last inspection in August. Supervision vers complimented on the orderly appearance of their office. The incident of the dropping of a snatch block was called to the attention of personnel as the type of unsafe work performance which if noticed could easily be prevented by the "I am my Brother's Keeper" type of action. The most recent report of inspection of safety equipment by department supervision was checked. The area was inspected from the ninth level on down. Items of housekeeping or safety noted for correction were as follows: 1. Buckets under EB sample taps on the ground level east of the elevator were left full of EB. 2. Several plug cock wrenches were loose on decking. 3. A wooden ladder without safety feet, mounted at too small an angle to the vertical, and too short for adequate contact at the upper end was observed on the ground level east of the elevator. It was removed immediately. 4. The condition of 3ome decking, kick plates, and rail ings Indicates need to expedite the approved replace ment and/or repair. 5. The 55 gallon drum used in conjunction with a completed trial at 7^8 should be removed. 6. An effort should be made to minimize the length of the power and ground wires to the refractometer in the laboratory section of the control room. Department 6-l6 This area was inspected on both levels. These personnel were told that the area showed good improvement vs. the last previous inspection in August.
