Your firm has been selected to participate in this survey because of the answers given on the initial medical surveillance survey. Responses to questions asked on that survey indicate that your company is either doing more or conducting programs in a different way from most firms interviewed. The follow up survey is designed to ask more in-depth questions regarding your current policies and practices. While the survey is voluntary, your participation and cooperation in this effort will insure the quality and usefulness of the information collected. OSHA will provide a copy of the results to those firms who participate in the survey. To ensure confidentiality, the survey information will be stored in a data base that will not permit individual firms or their responses, to be identified. As a part of KCA's agreement with OSHA, they will not provide any firm-specific data to OSHA. The survey covers the following areas: Process Hazards Analysis, Flammables and Hot Work, Safety Rules, Safety Training, Emergency Procedures, and Contract Employees. We would like to collect plant specific information about your company's facility at: HWY 25, ABERDEEN, MS, 39730 would appreciate your forwarding this letter to the person or persons in your organization best suited to respond to the survey. An interviewer will be calling you within the next few weeks. We are required by the Office of Management and Budget to inform you that the public reporting burden for this collection of information is estimated to average two hours per respondent; in some cases it may take somewhat less.
