COMMENTS ON STATISTICS In addition to the statistics, it was pointed out that there was a rising trend in Chemical Contact, Lifting/Pushing/Pulling, and in Striking Against categories. 3. OLD BUSINESS a. Emergency Procedure Manuals -- Glass The revisions in the Emergency Procedure Manuals are virtually complete with the exception of a few sections that were in for typing and printing. b. Plant-Wide Alarm System Survey -- Triplett Results of the plant-wide safety alarm survey shows certain parts of the plant needing improvement. Engineering work orders have been requested to bring these areas up to standard. A recheck of the plant-wide alarm system will be made during the January test of the Emergency Procedure plan. Some concern had been expressed by a number of groups in the plant about the cyanide area alarms and how they are tested. These new area alarms are being installed and are expected to be ready for testing at the time of the next Plant Emergency Procedure Test. Plant-wide publicity will be given to the cyanide alarms, what they indicate, and actions to be taken when they are sounded. c. Respiratory Protection on Fit Testing -- Ryan The Procedure Bulletin for Respiratory Protection Procedures has been completed and most of the comments have been re solved. The SAC plans to review it in detail with Mike McCullough prior to releasing it to final typing for issue. Also included are plans for plant-wide publicity and train ing prior to implementing this new procedure. d. 1978 Property Protection Survey -- Glass The report of the 1978 SPP Survey contained a number of positive comments about the overall safety program and its effectiveness. Certain areas for improvement were identi fied. The report of the SPP Survey will be sent to members of the PMSB. 4. REPORT OF THE SAFETY ADVISORY COMMITTEE [SAC] -- G. T. Ryan The remaining items on the SAC committee agenda include the follow ing: 1. A request for revision of the rules regarding riding bicycles through operating units. Tank content signs 5. REPORT OF SAFETY AND HOUSEKEEPING ACTION COMMITTEE CSHAC) -- Triplett ; The Hourly Safety Committee's plans, programs, and progress were reviewed. A question of "How well we are following the Fire Entry Permit Procedures?" was raised by the Hourly Committee. A detailed review of the procedure revealed that there was room for con fusion about the intent of the authors; therefore, the SHAC is reviewing it with the authors to clarify some of the wording. It is expected that the Fire Entry Permit Procedure will be rewritten. Activities Status Report: This quarterly report of the SAC has been issued. Also remaining on the agenda is the Plant Breathing Analyzer and the Procedure for the use of hoses within the plant. Recommendations for these two items are expected to reach SAC in January. 6. REVIEW OF INCIDENTS Major Injury to a Pipefitter in Dept. 13 - A pipefitter was injured on the tips of second and third finger of his left hand when a high-pressure water cleaning "Mole" was withdrawn from a pipe run. The physician found no water penetration in either cut and returned the pipefitter to full duty; however the investigation indicated that a much more serious injury might have resulted if it had not been for the prompt and alert action of the other Mole crew members. The primary cause was the complacency on the part of the injured after making hundreds of runs in and out of pipes with the clean ing device. A contributing cause was the low visability of the tape marking the hose which shows how far to retract the mole before shuting down the pressure. Review of the cause with the injured and other craftsmen trained to use this tool is in progress. Corrective actions regarding the rotation of the person doing this work and proper marking of the hose is in progress. 7. OTHER SIGNIFICANT EVENTS 1) Decatur Fire: A major fire occurred at Decatur when an AN pump ruptured due to vibration. The fire was under a pipe rack and was so intense that a high-pressure steam header failed and caused a one-week shutdown of the plant. Details of this fire are expected to be circulated at Texas City when the report of the fire is issued by Decatur. SC 14030 STG 2430875 LAM015819 PMSB MINUTES 12/20'^ Page 4 2) An incident occurred at Seal Sands in which the high-pressure reducing valve on a steam line failed due to sonic vibration. Details of this failure will be made available when the re port is received from Seal Sands. 8. CONTRACTOR SAFETY -- BILL RIDDICK Riddick reported that following the AN shutdown, a meeting was held with Opel Management regarding safety performance which resulted with a significant improvement during the styrene, acetic acid, and methanol shutdowns. This good safety performance was continuing at the time of the PMSB meeting. 9. OTHER ITEMS A. An operator received an exposure to TBA vapors when opening a tank truck hatch shortly after a transfer to the tank had been made. This was considered a Recordable Case because the operator developed chemical bronchitis and was given prescription medicine to relieve the condition but was returned to work without loss of time. B. Metten will review the procedures to determine what might be done to remove unneeded pipes from the pipe racks as a part of revision or modification projects in which retire ment of capital assests is involved.
