Kos This morning Department 15 had an injury caused, in part, by an improperly directed drain line. A quick survey of the surrounding area revealed several other such "traps." The problem is that the drains are directed so that they present a hazard to a person in a normal valve opening position. Please ask your supervisors to specifically check for hazardous drains in their daily rounds and scheduled safety inspections, and initiate action to correct any deficiencies found. Steam trap discharges (remember we are phasing out thermo dynamic traps), utility stations (need double block and bleed), and crowfoot couplings on process or utility lines (except air and cold water) are problem areas also that need attention. Crowfoot policy is that you can use them for decontamination on an exception basis, with the proviso that they are re moved immediately after use and that the connecting hoses are properly disposed of. Glass Visitor: Charles Goode (part-time) ANNUAL INSPECTION The video tape containing Phil Brubaker's Inspection Kick-off message was played with Charles Goode explaining operations of video tape equipment to committee members. This tape will be presented by SHAC members at departmental meetings. Lacy mentioned items which should be covered in these sessions and is preparing a more detailed outline. Slides containing examples of photographed housekeeping deficiencies are available from Arnold Glass. Stacy reviewed plans for guest inspections (scheduled for week of August 30) and is working on the tour schedule (proposed first day schedule was distributed). Cavender reported he had most of the required booklet pictures and has the majority of information requested from superintendents in hand. SAFETY CONTACT '76 Lacy reported that Safety Topics for assignment to experts had been received from the PT Safety Committee, the Office Safety Conmittee, and the Hourly Safety Conmittee through Jim Horan. (Topics had been received from all units and crafts by the end of the day, Friday, August 27, and approved topics developed by August 30 J Rather than select only a few of the numerous excellent topics, SHAC is asking Contact Supervisors to assign topics based on the ones already submitted. TASK FORCES A. Life Support Systems - Stacy The group met on August 19 and again on August 27. A purpose and scope has been drafted, contents defined, and responsibilities assigned for preparing individual sections. While the new bulletin will not replace the existing bulletin on Nitrogen Systems (P-0126) the task force has discussed the issue of fittings on nitrogen systems and reconmends that Thor fittings and dedicated hoses be used for nitrogen systems. B. Flammable Liquids - Cavender An addendum to the draft of the bulletin has been prepared, outlining flammable liquid storage requirement for using department's benefit. The proposed P-Bulletin will be revised by September 15, to include: 1. Draft as originally submitted. 2. Addendum as outlined above outlining requirements for different-type liquid. 3. Statement outlining additional requirements requiring individual department procedures. C. Out-of-Battery Fire Permits - Lacy ~ This task force met an August 23 and will meet again on August 31. The force is reviewing the existing bulletin with, the intent of preparing an addendum spelling out the requirements for pipe rack permit work in a concise manner addressing themselves to correcting deficiencies in the current system. TOXICITY INFORMATION FOR SHOP PERSONNEL - Hebert Hebert pointed out that shops personnel need easier access to toxicity information about the chemical service of equipment (pumps, exchangers, etc.) brought to the shop for maintenance since complete decontamination cannot always be guaranteed. Equipment will continue to have logo attached indicating its chemical service. In addition to the toxicity manual in the shift office, a chart will be prepared containing sumnarized toxicity information on chemicals used in plant equipment. It was felt that all required information could be sunmarized on both side of a large piece of paper to be laminated and readily available at shop areas. Hebert will coordinate this effort utilizing expertise from LPS (Fox and Safety Inspector?), maintenance personnel, and secretarial assistance to handle this project. Such a list might be appropriate for posting in other areas. EMERGENCY PROCEDURE PLAN Lacy presented the items assigned to SHAC in connection with steps being taken to update the EPP. LAM026422 STG 2430373 SC 18759 SHAC MINUTES - 8/27/76 -3- (EPP Plan List of Concern, 8/13/76): Item 21 - Shelter Adequcy for Protection of People from Spill. Cavender will review individual department plans, consulting and coordinating these actions. He will work with Carlisle to insure plans and required actions are spelled out in people centers (office buildings. Research buildings, etc.). Items 24, 29, 30, 31 - Concerning Back-up EPP Center and Personnel Accounting Plans: Tissue will contact individuals responsible for applicable parts of the plan (Bolton, Roy Arnold, Peters) to determine requirements for a back-up center and to investigate possible locations. The desirability of a combination back-up center/reassembly point as well as the possible need for an alternate back-up point was discussed. After preliminary investigation, an action plan to implement will be reviewed at the next SHAC meeting (week of September 7). PT SAFETY CCMOTTEE ITEMS Lacy reviewed two items for SHAC from Buck Cooper from the PT Safety Committee: 1. Vent over unleaded gasoline tank - is it large enough for fire relief? Hebert will investigate. 2. Need for safety glasses in print shop. This has been brought up previously and required use of glasses would be consistent with rules in other areas. Lacy will review further. HAZARDOUS INVENTORY PROCEDURE Lacy presented the recomnendatians contained in the report of the August 16 ammonia incident for a plantwide procedure covering work near areas containing hazardous inventories. The coranittee felt that a separate procedure covering all possible situations would be cumbersome and would be best handled on an individual department basis. It was felt that this subject, and the importance of controls, can be addressed in the Fire Permit Bulletin revision. For work not ordinarily requiring a fire permit, establishment of work permit procedures (as required in several manufacturing areas) is considered appropriate.
