Ward - Danbury Subject: Texas Dept, of Health Inspection, July 12, 1991 At 9:45 am, July 12 1991, Ms. Barbara Key from Texas Dept of Health requested entry to perforin a routine inspection. After viewing visitor's orientation video, Ms. Key was escorted upstairs to J. R. Dement's office. Ms. Key informed the group (Dement, Jackson and Vicknair) that she wanted to inspect Dykon Services, Inc., the asbestos abatement contractor at Bldg 113. At 10:25 am, E. J. Vicknair and A. P. Jackson escorted Ms. Key to Bldg 113. The group was met by met by Dykon project manager, Dan Myrick, and job supervisor, Tom Tuck, Jr. Ms. Key began her inspection by viewing the employee records and monitoring results. The group walked through the jobsite and talked with the Dykon employees. The inspection concluded at 11:35 am. Ms. Key briefed the group on her findings: 1) all annual physical records for the Dykon employees were not onsite, and 2) the certificates of asbestos abatement training (by an accredited course) were not onsite. Dykon agreed to have the information faxed to Ms. Key. Ms. Key also suggested that all safety meetings be documented and maintained. Ms. Key indicated that she would formally document the inspection and send copies to Dykon and me. At 11:55 am, I escorted Ms Key out of the plant.
