Turner December 3, 1984 Subject: Disciplinary Suspension - Inattention to Operations At approximately 3:40 p.m. on November 28, 1984, VCM was released from the atmospheric vent valve on D-500 reactor while you were attempting to vent D-300 reactor to the recovery system. This spill occurred because the condenser and recovery valves were still open while you were dumping D-500 reactor. When I asked you what happened, you explained that after you finished recovering D-500, you placed the reactor in the dump mode and opened the main dump, atmospheric vent and slurry valves. You said that you then ppened the condenser and recovery valves on D-300 (which was in the emergency mode). When you heard the VCM venting, you immediately closed the valves on D-500. You could specifically remember that the panel light indicated that the condenser valve was open. Immediately following this incident, we were unable to duplicate the conditions that you recalled. The only mode in which the condenser, recovery and atmospheric vent valves would simultaneously open on D-500 was the emergency mode. An interlock check-out by instrumentation and engineering personnel revealed no problems. The lead operator is responsible for the safe, efficient, and environmentally sound operation of this unit. This VCM release would not have occurred if you would have .verified that the condenser and recovery valves were closed before you opened the atmospheric vent valve. I am placing you on disciplinary suspension for two (2) days; Dec. 10 and 11, 1984. You should return to work on your normal schedule on Dec. 12 at 7 a.m. I hope that you will take this opportunity td reflect upon your job respon sibilities and the importance of closely monitoring reactor instrumentation. Any future incidents of this nature may result in more severe disciplinary action up to and including termination of your employment. C. W.
