If you prefer, you may sign the enclosed Permission to Release Death Certificates for an Epidemiology Study from ENSR to AEI. An NDI Application to update the Vinyl Chloride Epidemiology Study from 1983-1992 has been submitted to the National Center for Health Statistics. Upon receipt of the NDI file search results, a request for additional death certificates along with the appropriate fees will be sent to you.
