Main Street Duchesne, Utah 84021 Facility Phone: Facility Representative: EPA Facility ID Number: 5040026 Reason for Inspection: Routine compliance inspection Inspector(s): Marta Grabowski, EPA Andy Woodward, EPA UST System Description The Fast Stop Travel Center 1101 (Facility) is a privately-owned gas station and convenience store located on the Uintah and Ouray Reservation in Duchesne, Utah. The fuel at this Facility is sold to the public. The Environmental Protection Agency's (EPA) Facility records indicate that the Facility has three, single-walled, lined steel underground storage tanks (UST). The internal liners were installed circa 1996-98. Tank 1 and Tank 2 have a capacity of 8,000 gallons, and Tank 3 has a capacity of 6,000 gallons. Tank 1 is empty and has been out of use since December 2014. Tank 2 and Tank 3 store regular unleaded fuel and premium unleaded fuel, respectively. Facility piping is double-walled, pressurized flexible plastic. Inspection Narrative On June 27, 2023, the EPA UST program personnel conducted an inspection at Fast Stop Travel Center 1101 in Duchesne, Utah. The EPA last inspected the Facility on August 6, 2019, and they selected it for routine re-inspection. Ms. Grabowski spoke with the Facility Operator on June 20, 2023, and the Facility Manager on June 12, 2023. She also sent follow-up emails to the Facility representatives to schedule the inspection (see Attachment A). 5040026- Fast Stop 1101 Page 1 of 3 6/26/2023 UST Inspection The EPA inspection team arrived at the Facility at 2:30 p.m. MDT. They were met by the Facility Operator. Ms. Grabowski introduced herself and presented her inspector credentials. Ms. Grabowski filled out a Notice of Inspection (NOI) (see Attachment B) and she requested the operator's signature. Ms. Grabowski explained that the NOI serves as documentation of consent for the EPA to conduct an inspection. In addition, she said that she would provide the NOI to the Facility at the end of the inspection. EPA inspectors conducted a physical inspection of the UST system as well as a review of Facility records. The EPA inspectors took photographs of UST system components during the inspection (see Attachment C). Copies of operation and maintenance records provided by the Facility at the time of inspection are in Attachment D. Tank Leak Detection Tank leak detection for the regular unleaded tank is achieved via statistical inventory reconciliation (SIR) in conjunction with a Veeder Root TLS300 automatic tank gauge (ATG). The ATG conducts Continuous Statistical Leak Detection (CSLD) on the premium tank. Both active tanks had 12 months of passing leak detection results on file at the Facility. Annual functionality testing for the automatic tank gauge (ATG) was conducted and passed on October 28, 2022. The MLLDs passed annual functionality testing on October 28, 2022, and both lines passed annual line tightness testing on October 28, 2022. Cathodic Protection Cathodic Protection (CP) for the tanks is provided by an internal epoxy tank lining, which should be inspected every five years for each tank. The Facility operator showed a hard copy of the internal lining inspection report from December 2014, and she emailed the September 29, 2019, internal lining inspection report the day after EPA's onsite inspection. All tanks passed inspection in 2014 and in 2019. Periodic Testing/Inspection of Spill and Overfill Prevention Equipment According to Facility records, the spill buckets were hydrostatically tested on October 28, 2022, and the overfill alarm was tested on August 18, 2022. All spill prevention and overfill prevention equipment passed. Periodic Operation & Maintenance (O&M) Walkthrough Inspections Records of monthly walkthrough inspections for the last 12 months were provided at the time of EPA's inspection. The Facility operator conducted an annual walkthrough inspection on June 26, 2023, prior to the EPA inspection. 5040026- Fast Stop 1101 Page 2 of 3 6/26/2023 UST Inspection Operator Training Class A/B operator training certification was maintained onsite, in addition to a list of Class C operators and their training dates. Financial Responsibility The Utah Department of Environmental Quality issued a 2023 Certificate of Compliance to the Facility, indicating that the Facility is registered with the Utah Petroleum Storage Tank fund (fund). The fund serves as the Facility's UST financial responsibility mechanism. Physical Observations The inspection team viewed the spill buckets/fill pipes, earthen submersible turbine pump (STP) sumps, earthen dispenser sumps and the ATG console. The fill pipe for Tank 1 was locked and the Tank 1 STP was bolted and inaccessible. Spill buckets for Tank 2 and Tank 3 were dry. A manifold access point was present; however, it was no longer in use according to the Facility operator. MLLDs were present for Tank 2 and Tank 3. Additionally, an audible and visual overfill alarm was noted on the side of the building next to the tanks. Inspection Conclusions and Recommendations Prior to departing the Facility, Ms. Grabowski stated that a full inspection report would be forthcoming, which includes the inspection checklist (see Attachment E).
