The U.S. Army Corps of Engineers (" Corps ") has notified the EPA of these work activities on your property located at (b) (6)Drive, Greenbank, Washington 98253, Parcel No. (b) (6) Lot (b) (6) Latitude (b) (6)and Longitude (b) (6), on Whidbey Island in Island County, Washington (" Site "). The EPA requests your voluntary cooperation in providing the following information to help us better understand the nature of your contractor's work activities: 1. Depict on a map or survey the location of the Site where you and / or your contractor placed any fill material and where any other work activity has taken place. To the extent you and / or your contractor conducted or hired any consultants to conduct high tide line delineations, depict the delineation on the same map and provide any supporting documentation and / or analysis. 2. Describe all work activities you and / or you contractor conducted at the Site. For each discrete work activity, include in your description the following information: a) approximate commencement and completion dates of the work activity. b) approximate area or acres, including linear feet and width disturbed and / or impacted. c) approximate volume / cubic yards of dredged and / or fill material excavated and / or discharged, including linear feet, width, height and / or depth disturbed and / or impacted. d) the source of the dredged and / or fill material and provide the source location on a map. e) the purpose and need for the work activities. f) the person(s) requesting the work activity and, if different from the Site owner, their contact information. g) the person(s) receiving payment for the work activities and their contact information. h) the person(s) providing payment for the work activities, the amount paid and, if different from the Site owner, their contact information, including name of the company, name of the manager, mailing address, email address, phone number, etc. i) the person(s) performing the work activity, whether they received compensation or not, and their contact information, including name of the company, name of the manager, mailing address, email address, phone number, etc. j) the machinery and heavy earthmoving equipment used to perform each work activity and the name(s) and contact information of each person that operated the machinery and / or equipment. k) the ownership status of each piece of machinery or equipment identified in C.2.j, above. For each piece of machinery or equipment that is not owned or financed by you (e.g., rented borrowed, provided by contractor, etc.), include the name(s) of the owner(s) and their contact information, and information about the dates of use. I) the sequence of work activities conducted at each Site. 3. Provide copies of any and all correspondence with your contractor(s), including but not limited to documents related to permission to access and conduct work activities in the Site or pertaining in any way to the placement of dredged and / or fill material at the Site, including whether or not a CWA Section 404 permit was required for the work activities and, if so, whether you or your contractor would seek permit authorization. Describe in detail any discussions you had with your contractor regarding CWA Section 404 permitting for the work activities at the Site. 4. Provide copies of all sketches, plans, and as - built drawings related to the planning, conducting, and completion of the work activities at the Site. 5. Provide any additional information that may be relevant to this matter. If possible, it would be appreciated if you could provide this information to the EPA within 30 days of your receipt of this letter.
