< Interoffice Communication To Don Johnson From Roger Collins Date August 3, 1976 Subject VCM RELEASE VIA BLOWN SAFETY DISC D-500 D-500 reactor was charged at 1425 and was heating up when the southwest safety disc on shell blew. The relief valve lifted also The product was 5305 with set point at 150, The reactor temperature was 149 and the pressure 141# when the safety disc blew at 1540, Panel Operator, Bobby Chaffin, immediately put instruments in manual and put full cooling on reactor, started recovering and sent Bob Newman out to kill reaction with 3 gal. AMS. At this time, the VCM vapor release number was dialed and location of release given. Dryer Lead Operator, R. C. Towery, shut dryers down via emergency shutdown switch. Relief valve reseated approximately 3 minutes after it lifted. Pressure had fallen to 130#'s and temperature to 140. Lost approximately 2,500 #*s VCM, The wind was out of the northeast, therefore, directing the VCM release to the lagoons and open field area. After letting the Honeywell make a full cycle, and no VCM detected, the all clear number was dialed and normal operations resumed. No one was exposed to VCM. As far as I know, all vehicles stopped, with the exception of a Jesco Contractor. Mr. Bob Seymour reported these to me later and said he would give Jdhn Dixon a call and have him go over safety with these people. After the reactor was recovered and dumped, I had maintenance pull the blown safety disc, insert a skillet under the relief valve and pressure test it. It lifted at 175# which is the proper setting. I then had maintenance change all safety discs on this reactor to insure their reliability, I then told the panel operator to pressure check reactor and charge if OK. I also told operator to charge reactor with set point 5-8 below formula set point and run up when lined out. Also to check pressure gauges on reactor to see if they corresponded with the pressure on the panel board. R. Collins c: ELK, GGD, JVU, RWS, JEB VAB.0001220797
