The 3rd Quarter Safety & Housekeeping Inspection for the HCN Complex was conducted by the inspection team of Ray Forsthoffer, Bob Bohmler, Dan Nelson and Bob Hammann on August 13. The unit was given a qualified "good" rating. The reason for this rating was the shutdown work in progress.
The following items need to be corrected:
Safety
1. Recommend making safety procedures more visible (presently in cabinet).
2. Training manual was lagging in some areas.
3. Some old papers and signs on bulletin board - update?
4. Numerous hose inspections not up-to-date.
5. Safety chains missing from a few chain operators in rack - storage area.
6. HCN storage area:
a) Other items stored with acid in bottom shelf of cabinet. b) Nitrogen hoses are tripping hazard at safety shower and eyebath south of
TIO's. c) Half of a Dept. 51 extension ladder is by the emergency generator.
This ladder is damaged and needs to be destroyed.
7. Hose at safety shower/eyebath location, north side of AN shack is a tripping hazard.
8. Men were working on the north end of 27T1 without a scaffold.
9. CT2:
a) Inspection ports need to be repaired on shrouds at CT2-7.-3, and -4. b) Shroud retainer ring broken on CT2-5 - also need to check all other
retainer rings due to corrosion on bolt assembly. c) Electrical plug roped to railing between CT2-1 and 2 on east side.

d) Sign on entrance to stairway on south stairs is broken. e) Chlorine line below block valve at chlorinator looks to be in
poor condition.
Housekeeping
1. Trash in and around extinguisher cabinet No. 8 by 25T12-2 dike wall.
2. Trash in T10-4 pit and on catwalk south of T10-2.
3. Need general cleanup of the TBA-LN storage tank pits.
4. Buckets, mop and various and sundry trash in CT2 breaker room.
5. Remove old refrigerator from kitchen area.
6. Maintenance work area needs straightening up.
7. Scaffold along side D101 should be removed.
8. Trash around 972T2-5 & 6.
