Safety Zone I was inspected by the Union Representatives with items noted being carried forward as open items. Several general questions were asked by the Union Representatives as follows: 1. Has the Company started First Aid Training and, if not, when will this training begin? 2. Has the Company set a target date for completion of covering exposed asbestos within the Plant? What percentage of this work is completed in the Plant? How many people are working on this job at the present time? 3. It was noted that there are scaffold planks in the Plant that have no indication of being tested and marked this year. Some are in use. When will these scaffold planks be tested and marked? 4. Is the size of the off~shift fire crew adequate? How many people are available for this crew and which unit does each come from? Is the company aware of any industry standards or rules of thumb on this subject and, if so, how does PSP compare? 5. It was noted that an MSDS book was not available on the VPS unit concerning chemicals used on the unit. Can this information be made available to each unit? Several additional items of a general nature were discussed by the Committee. Present insulation loose and falling apart. CES-Invest. & Recommend DCU-Investigate replacing bottom straps used for closing doors on elevator. Also noted no state inspection posted in elevator. CES-Invest. & Recommend DCU-Investigate. use of insulation diapers on 18" wedge plugs at third deck of coke drum structure. This type of installation proved to be fire hazard on VPS. CES-Invest. & Recommend DCU-Investigate sampling of coke dust throughout the DCU to determine if there are any associated health hazards of personnel breathing dust. WAR-Invest. & Recommend DCU-Investigate the furnishing of cover alls for Operators on the DCU* CES-Invest. & Recommend General-Investigate use of H2S monitors by personnel working in areas of potential H2S exposure.
