Type and Purpose of Inspection Reconnaissance without Sampling IV. Facility Description The facility is a construction site, apparently for a large commercial building. As the area is being redeveloped and streets rebuilt, streets and names may be somewhat in flux. Using street names given by Google Maps as of this date, the site is located west and south of Hospital Drive, north of Dorista Goldberry Drive, and west of Clocktower Drive. The terrain slopes generally down to the south and slightly down off the site to the east, and mostly up off the site to the west and north. At the time of the inspection a foundation was in place in the center of the site, and three steel beams on the foundation, but no other structure. A small basin was in place in the southwest corner of the site. No personnel were present, and it did not appear that work was being actively performed. V. Inspection The inspection was performed as a reconnaissance inspection; the inspector did not contact site personnel or enter the site. All observations were made from public roads or during a separate inspection of the road reconstruction project adjacent to the site. Mr. Spejewski and Mr. Flint arrived at the site at 1:00 PM. They walked around the south and east sides of the site on the public road, observing the site. After this reconnaissance of the Galaxy site, Mr. Spejewski and Mr. Flint then performed a separate inspection of the City road reconstruction to the north and west of the site. From that project, they observed the Galaxy site and took one picture of the Galaxy site. That picture was taken after several pictures were taken of the road project, leading to a gap in picture numbering for this inspection. A. Observations The site was surrounded by chain - link fence. The center of the site was a large flat area, with concrete walls (apparently a foundation) surrounding it. Inside the walls appeared to be bare dirt with many round concrete pads installed. Three vertical steel beams were installed at one end of this area. West of this foundation was some apparently pre- existing pavement (with old, faded parking lines painted) and a foundation, parking / staging area of new asphalt. 2 ED_019088A_00019851-00002 Most of the remaining western end of the site was unstabilized bare dirt.A small basin with standing water was in the southwest corner. Erosion gullies a couple inches deep led into the basin. The south border of the site was a slope down (about 10-15 yards) to the pre - existing paved road. The slope was well grassed. Silt fence was in place along the foot of the slope, just before the curb of the road. In the center of the south edge was a round concrete structure; rip - rap was in place between the structure and the road. A catchbasin was in the road just below the rip - rap area. A silt sack was in the catchbasin's grate, but did not block the opening in the curb above the grate which also led into the catchbasin [see picture 2666]. The silt sack was showing signs of wear. The eastern end of the site was mostly bare dirt without any visible stabilization. There was scattered weed growth, but not enough to stabilize the soil. The southeast corner, which sloped down to the road in both directions, was covered with rip - rap stone. The east side of the site was a slope down to the road, about two yards high. It was mostly unstabilized. A line of silt fence was in place at the foot of the slope. The northern edge of the site sloped up to the north. A row of trees and grass was along this edge of the site.
