Baker poin^e^oul^h^/^a^^ty Sampling is conducted monthly in each of the depwrtratats in your plant* Each fore man checks the working practjxcehNtf eatfi [Employee and each operation In completing the Safet^Ssapling of his area of supervision* As a result of this procedure, the foreman is able to pick out accident exposures and unsafe-practices* Preventive measures are then to be init; INDUSTRIAL HYGIENE RECOMMENDATIONS Mr* Baker indicated that the installation of exhaust equip ment is scheduled for the Brook Press - Preform area within the next thirty to sixty days* This additional equipment should minimize the asbestos exposure in this area ofyour plant* ACCIDSKT INVESTIGATION - BLOK PRESS DEPARTMENT Hr* Baker, Mr* Roberts, and I investigated the circumstances involved in the 5/15/63 accident to Mr. Walter D. Bovtn in the Blok Press Department* Mr* Baker pointed out that the accident was produced when Mr* Bowen stepped on the stripper treadle and caught his finger between the stripper and the upper plate as the die was being stripped* The die has since been guarded to prevent this type of accident from occurring again in the future* SPNY 005572 Page 2 August 7, 1963 Following our discussion on the above natters, an accident prevention survey was conducted in the plant* The result of this survey was discussed with Mr* Baker, including several minor recommendations* The most serious accident exposure noted is discussed on the attached Recommendation Sheet* Very truly yours.
