Chairman Manuel Perez opened the meeting introducing Greg Daues as guest speaker. Greg discussed the STOP program for hourly personnel. He explained the purpose of STOP which is to eliminate unsafe acts, thus reducing injuries. He discussed the mechanics of the program explaining that the green cards were not meant to point a finger at someone, but were used to collect data identifying problem areas. Greg reported on the current STOP training for Chiefs and Leadmen. He asked the HSC for two committeemen to help prepare the version for journeymen and appren tices. The OSC will aid in preparing a STOP course for lab technicians. Greg ended by commenting on the improved safety performance of the plant and attributed this to everyone doing their job for safety. Rufus Baca, representing SHAC, reported on the following unfinished business: Cooling Tower Fan Hazard When the fan drive motors are shut down, the fan continues to turn due to wind drafts through the tower. Attempting to stop the fan for repairs is hazardous. 4$ ^ cf fc.L. j Hl^ll Action A design for a brake is complete and will be recommended to the PMSB. Removing- Bolts from 51D4 Manway Prior to Decontamination A committee member reported that during the last AN shutdown, all but four bolts were removed from each manway prior to blinding the column. coMpWedl- don't eh jaUo SI4A Action The plant has been surveyed for current practices regarding the opening of vessels for non-entry inspections without blinding and the results given to SAC for further action. LAM013498 Barricade Tape A concern was voiced about barricade tape -- tape is being left up after the need no longer exists. Therefore, many times tape is ignored. Action In addition to the letter sent to plant superintendents restating current procedures for barricade tape, the SHAC representative pointed out that we all have to do our part in correcting the abuse of barricade tape. He suggested that when barricade tape is found and suspected to be outdated, contact the owning department and bring it to their attention. .Radio Calls Waldo Hughes stated that it is not always possible to respond immediately to radio calls from their supervision and that some supervisors continue to call repeatedly. This in itself does not necessarily create a safety problem, but the repeated calls that result when one doesn't answer imme diately become a nuisance and a distraction. Action The matter was called to the attention of the Zone Supervisors. The SHAC representative also suggested that when this occurs, one should contact the caller and explain the problem to him. Plectron Checks Albert Solis raised the question as to who checks the plectrons and how often. Action A test will follow the 1:00 P.M. Fire & Spill alarm test on Mondays. NEW BUSINESS Hole-man Responsibilities A question was raised as to what are the responsibilities of a hole-man. Action Harry Conrad will head up a task force to define these responsibilities and guidelines. SC 12303 LAM013499 Paint Overspray Covers Controls on Manlift J. W. Hughes pointed out that instructions for the controls on the painters manlift were covered with overspray. Action This was referred back to the Painters' Supervisor for correction. Samples Being Carried Up the Stairs to the Control Lab Lee Derrick questioned the safety of carrying samples up the stairs to the Control Lab. Action ^ *jor corner *PRJL This was referred back to the Unit Craft Safety Team for an answer. Tape Sleeves on Slicker Suit when Handling Phenol W. A. Lowery expressed a concern about the practice of taping sleeves of slicker suits when handling Phenol. This practice was discussed as a result of Phenol burn to a Pipefitter. Action The practice of taping sleeves of slicker suits will be written in the procedure. Use of Red Barricade Tape A question was raised as to who uses red tape. Action Mark Riddle explained that it was primarily used by the Fire Department and would not be used by the plant, so people would not lose their respect for the tape. LAM013500
