January 15, 1988 To: The Vinyl Institute Health, Safety & Environment Committee Re: OSHA Recordable Incident Rate Data - Calendar Year 1987 For the last two years, the Vinyl Institute has collected OSHA Recordable Incident Rate data and presented awards at the Vinyl Institute Annual Meeting to the first and second best perform ers in the monomer/EDC category and in the PVC category. It is now time to gather data for awards to be given in conjunction with the 1988 Annual Meeting (May 11-13, 1988 in Scottsdale, Arizona). Please return the attached sheet with your calendar year 1987 data directly to me by March l, 1988. (As noted, the data should be provided on a plant-by-plant basis). From the data submitted, we will determine the "winners" at the Committee's March 8th meeting. We would like to have responses from all of you so that we can calculate an overall rate for Vinyl Institute members to show how we compare to various segments of the manufacturing industry. January 15, 1988 To: Vinyl Institute Executive Board Vinyl Institute Health, Safety & Environment Committee Re: Annual Vinvl Institute Award For Contribution To Safety In 1984, the Executive Board approved the establishment of an annual award for contribution to safety. This award is meant to recognize the most valuable contribution to safety in the vinyl industry that has been made recently. Such a contribution may have been made available to the industry in general or may have been a unique program in an individual company that has proven successful and which that company now wishes to share with the industry at large. Nominations should be sent to my attention and received no later than March 1, 1988. A suggested format is attached, but nominations can be in any form (such as a letter) provided the requested information is |Contained therein. The Health, Safety & Environment Committee will evaluate all nominations and make its recommendations to the Executive Board for a final deci sion. The award will be presented at the Vinyl Institute Annual Meeting in May 1988. You will recall that previous awards have gone to the following recipi ents: 1985 - Dr. John Stafford and ICI for the establishment and maintenance of the Angiosarcoma Registry. 1986 - Dow Chemical for a presentation on worldwide storage and loading facilities. 1987 - Mr. John Barr and Air Products for unique contribution to industry publications and to standards setting and safety organizations.
