Survey Work Conducted at Information Center 1973-82 5/11/73 - Preliminary noise survey of equipment room conducted by 0. P. Atwood. 6/8/73 - Noise survey was conducted by D. P. Atwwod in EAM Room (2P24), Key Entry (2W23), Commercial Computing (1P09), Production Scheduling, Technical Computing, Mail Room (1A05), and Decollating and Bursting Room. Survey was requested by Information Center Commercial Computing Division - Data Processing and R. L. Jeffers, Personnel Services. 6/14/73 - Noise survey of Addressograph machine located in the Fannin Bank Building was conducted by D. P. Atwood. This equipment was scheduled to be moved to the Information Center. R. L. Jeffers, Personnel Services, requested the survey. 7/13/73 - Octave band analysis conducted by D. P. Atwood to determine whether or not proposed audiometric test room met require ments set forth in ANSI S3.1-1960 (R-1971). Request was made by R. L. Jeffers, Personnel Services. 3/15/74 - Noise survey conducted by D. P. Atwood in Technical Computing Division's terminal rooms in the basement and on the seventh floor. Measurements were also made in the basement mechanical equipment room. Survey was requested by R. L. Jeffers, Personnel Services. 10/2/74 - Noise dosimetry was conducted by D. P. Atwood on the operators and maintenance men working in the engine room of the Information Center. This was a follow-up to earlier sound level surveys. The results ranged form L0SHA=89.6 (Senior Operator) to L0SHA=75.3 (Maintenance Man). 11/8/74 - Letter from J. D. Ransdell to R. L. Jeffers providing information on a variety of hearing protectors. 11/11/74 - Noise survey conducted by J. D. Ransdell in the Technical Operations room, Environmental room, Terminal rooms, CDC Operations room, and the bursting and decollating room. This survey as intended to supplement data reported in 3/15/74 report and to provide comparisons between Univac and IBM equipment. ^S-011763 LAIM 031820 2- - 1/7/75 - Letter from J. D. Ransdell to C. F. Phillips summarizing noise work conducted at Information Center to this date. 1/23/75 - Project 7-7-74, noise survey follow-up, TCD was transferred to C. F. Phillips. Included in project file was a draft memo to Manager - Technical Operations submitting a schedule for personal noise dosimetry monitoring in the TCD terminal rooms. 2/19/75 - Memo from Manager - Purchasing & General Services, Information Center to Maintenance, Operations, and Utility Personnel establishing a hearing conservation program. 6/16/75 - Memo from R. L. Jeffers to D. P. Atwood dated 4/4/74 was sent to C. F. Phillips for action. The memo requested an evaluation of ozone exposure in the film processing room of the Technical Computing Division. 10/10/75 - Report entitled "Noise Survey, Shell Oil Company, Information Center, Houston" (Project 75-402) was issued. Report included dosimetry and octave band analysis for mechanical equipment areas, emergency generator building, TCD, CDP, basement terminal rooms, and the CMTC area. 1/12/76 - Noise survey conducted by C. F. Phillips in Rooms 1P43 and 1P33. Rooms contained decollators, bursters, and printers. Need for reevaluation indicated if OSHA standard is lowered to 85 dBA. 4/8/76 - Noise survey conducted by C. F. Phillips in first floor mail room (1A05). No levels above 90 dBA found. 1/14/77 - Noise survey conducted by C. F. Phillips in Rooms 4P33, 4P83, 2P16, and 1A05. 4/25/77 - Memo from C. F. Phillips to R. P. Dennis, Employee Relations, evaluating potential employee exposure to polychlorinated biphenyls and procedures for safe handling and disposal of PCB's. 7/28/77 - Memo from C. F. Phillips to L. B. Kerr recommending location for high noise area warning lines in chiller area at the Information Center. 12/5/77 - J. F. Byrne conducted mock OSHA inspection. This was done to illuminate hazards and promote safety awareness. Several action items were implemented by T. J. Ryan at this time in an effort to improve the overall safety record at the Information Center. ABS-011764 LAM 031821 -3- 1/30/78 - Note from C. F. Phillips to R. Dennis referring him to Subparts D and E of 0SHA standards for information on platforms. 2/3/78 - List of chemicals used in film processing area sent to J. P. Murphy and T. J. Dorsey. 2/6/78 - Notes from J. P. Murphy to C. F. Phillips indicating odor problem with Versatic printer and report of significant hearing loss in employee located in Room 1P04. 9/12/78 - Noise survey conducted by J. B. LeFever in certifier and terminal rooms and areas of Com Processing and On-Line Systems. 11/13/78 - Evaluation of dust exposure in the Data Evaluation Department was made due to employee complaints. Dust source was the computer tape disc conveyor (Aggie Swing). Specific recommendations for clean-up were made by J. B. LeFever. 6/8/79 - Information on Halon 1301, used in fire protection system at Information Center, sent to G. Coleman by J. B. LeFever. 7/24/79 - Memo from J. B. LeFever to G. Coleman explaining Shell's position on use of Company vehicles by hearing impaired employees. 2/13/80 - Report from W. B. Austin to J. J. Marsh concerning potential inhalation hazards following a fire and release of Halon 1301 in the computer rooms. 12/18/80 - Noise survey conducted by W. B. Austin in printer room at Buffalo Savings Building. Information Center employee was concerned about noise exposure. Highest noise level measured was 77 dBA. 3/4/81 - Noise survey conducted by W. B. Austin in Technical Data Processing Department (1P18). Highest noise level measured was 77 dBA. 9/10/81 - R. J. Duhe conducted a chemical hazard review for chemicals used at IC. 12/28/81 - Noise survey conducted by W. B. Austin in BA-31. Noise source was new IBM 3211 printers. Results ranged from 64 to 76 dBA. 1/25/82 - W. B. Austin monitored 2P18 for trinitrofluorenone used in the IBM 2800 printers. No TNF detected. ABS-011765 LAM 031822 -4- 3/11/82 - W. B. Austin performed noise survey in 8A06 and 9A18 where Harris printers were in use. Noise levels ranged from 75-80 dBA and 80-82 dBA, respectively. 8/20/82 - Noise survey conducted by W. B. Austin in building services group. Several high noise areas identified. Noise dosimetry to be scheduled. 9/10/82 - Chemical hazard evaluation made by W. B. Austin for employees operating Versatec plotters in 1P36. 10/15/82 - Noise dosimetry conducted by W. B. Austin for building services group. Results ranged from 74.2-83.5 dBA L0SHA. 10/15/82 - W. B. Austin and Q. J. Machac conducted Health and Safety review for entire I.C. Several recommendations made.
