ASBESTOSIS : Baader E. Prof. W. D. Berlin Pre : i Director University Institute for Occupational Diseases * Lecture presented to the meeting of the Berlin Medical Society on November 1 1939 e OCCUPATIONAL J OCCUPATIONAL OCCUPATIONAL OCCUPATIONAL HEALING OF AND ENVIRONMENTAL HEALING CANADA INSTITUTEINSTITUTE ENVIRONMENTALCANADA MONTREAL 1835 K Mineral asbestos is a magnesium salt of silicic acid that contains no free silicic acid It is widely distributed on earth the principal | deposits in Canada South Africa and the Ural mountains In the ~ Greater German Reich there is a workable deposit in Rechnitz Burgen- land a few kilometers from the Hungarian border The mineral is mined everywhere by strip mining It occurs mainly in two forms as rather inelastic hard hornblende asbestos rich in silicic acid and lime but poor in magnesium hornblendies mined principally in South Africa and Italy northern ayy The other form is the softer serpentine asbestos which is pooreirn lime but richer in magnesium and water serpentine is ex- tracted mainly in Canada and Siberia After being quarried both forms of asbestos are crushed and carded rr, that It is the fact it can be separated into fine rock needles that has . led to the mineral's industrial uses particularly since it offers several remarkable qualities extremely high fire resistance resistance to acids thermal insulation and electric conductance The fibered needles of serpentine asbestos are so soft silken and elastic that they have earned the name mountain flax they can indeed be spun and woven like flax Mountain flax is actually used for textiles clothing items for firemen thermal insulation material hoods twine etc. The fibers are first loosened by carding then passed through spinning frames or mules and finally conveyed to the loom The carding and weaving causes dust The fiber needles of hornblende asbestos are not as suited for spinning they are used as raw material for all sorts of combination products such as asbestos board asbestos mortar or as additives to increase the strength and toughness of syn- thetic resins Asbestos mixtures are also used for automotive break linings Asbestos is very frequently mixed with cement to make build- ing panels for roofs and exterior walls - I need only remind of the Eternit works located in Berlin - as well as pressure gaskets and seamless pipes over steel cylinders it is used to manufacture vats casks and tanks for the storage of acids wo eee cement construction panels the grinding The sawing to size of asbestos filing and fitting of acid- proof containers and of finished asbestos cement molds are all sources of dust production Since asbestos cement contains only 10 asbestos its dust naturally does not yield as much asbestos as the dust pro- duced in the asbestos mill or more particularly in a mattress re- se pair shop . . Asbestos whose industrial exploitation only began in the second half of last century has gained rapidly growing importance By 1928 world consumption was five times that of pre WW1 and it has continued to grow apace in the last decade There are today an estimated 800 people in Germany who work in the producing industry Just as with silicosis the danger of unprotected work in the asbestos industry was only recognized fairly late It is true that already in1900 the Englishman Murray had characterized as occupational disease the pulmonary fibrosis of a year laborer who had worked for ten years in an asbestos plant during autopsy Murray had detected asbestos needles in the lung but this valuable pioneering study re- ceived practically no recognition at the time In Germany it was 1931 ~ _ Qe before von Buttner and Trillitzsch described for the first time a case of asbestosis in the industrial region of Saxony under the name mountain flax lung Since then and particularly particularly in the last few years there has been dilligent research and reporting regarding this new occupational disease In this connection I would like to call the ray 1938 attention to excellent ome. eee atlas of asbestosis by Saupe asbestosis silicosis course The se ce ee of does not differ from that of ; which has been described described for us by B^hme only the finest rock dust is inhaled during work in asbestos plants On the other hand the - pathologic processes - =. of asbestosis and silicosis are very different from other Asbestosis is characterized by a diffuse fibrosis of the pulmonary tissue that recalls chronic indurative interstitial pneumonia - direction The changes become increasingly pronounced in a descending Pure asbestosis never displays the nodulation or keratosis found in silicosis When it is observed it is always found to be a case of asbestosilicosis since it is not too rare for asbestos workers before to have worked on materials containing silicic acid containing I do not need to dwell on the clinical aspect of astestosis since principal symptom silicosis its asbestosis symptom cannot symptom is shortness of breath just as it is in be diagnosed with enough certainty by either percussion or auscultation without an ray Matching the anatomic findings the ray picture of asbestosis also differs fundamentally from that of silicosis But I must point out that on occasion the ray may not reveal any noticeable change despite a patient's vocal and entirely believable complaints this applies even when one can can reject out of hand the possibility of any other disease that might cause shortness of breath oppressive feeling and coughing In general the ray picture produced by asbestosis is a much less striking one It characterized by exceedingly fine network and reticulation with an embedding ofequally fine and delicate maculation Transverse and ; _ diagonal strip shadows appear quite early particularly in the formed angles by the chest wall and the diaphragm In general the rod more areas are lower weertes stce shaded than the upper lung regions which not infrequently display heavy. emphysematous changes and are often radiolucent in silicosis on the contrary it is the basal region of the lung which usually becomes lighter because of emphysema In general there is a more pronounced expansieon xpansion of the hili along the heart rim The asinduced changes are often more developed on the right side as Saupe points out | Wedler who ran serial tests at the Siebeck clinic' found that his patients almost as often had a thickened left side In advanced stages ribbon shadows are often observed around the heart rim they impinge on the heart contour like felt strips and make it look blurred It must be pointed out that with asbestosis the most difficult differential diagnosis is for pulmonary congestion which as is known also causes stronger heart markings shadows on the dependent lung segments and spreading lung structure created of the hili by asbestosis But the combed is considerably more meshed delicate and finer than that of pulmonary congestion Saupe and many others con- cerned with asbestosis found that diagnosis becomes less difficult -4- as experience is gained And one must always keep in mind that the ray picture is better at revealing the intensity of reaction than the gravity of the pneumoconiosis the lung tissue's The condition which distinguishes the asbestos worker from all others is the presence of extremely fine asbestos fibers in his body's discharges as originally demonstrated by Gloyne exactly ten years ago first he identified asbestos in the sputum and in the pneumonotomy later also in the lacrimae and the feces The fibers which are color- less transparent and birefringent are referred to as asbestos needles when a uncolored pneumonotomy of asbestlounsg is subjectetdo field illumination masses of asbestos needles will be seen to glow - Beintker found about 1,300,000 asbestos needles in 1 ccm In the human body these asbestos needles can change into peculiar structures a mantle of gel- like organic material will form around an asbestos core in contrast to the colorless asbestos needle this sheath is given yellow brown or dark brown coloration by trivalent iron These colorful structures take on the most diverse shapes looking like clubs dumbbells or spears sometimes the segmentations and constrictions of the mantle around the asbestos fiber makes them look like water lilies These structures which Fahr discovered as earlays 1914 are called asbestos bodies It is still not clear whether they originate in the lungs or in the upper respiratory tract Nor has the last word been heard about the mechanism by which asbestos bodies are formed The clever theories advanced by geologists Beger and Sundius are diametrically opposed to each other But But antiform can be used to enrich the asbestos bodies in the discharge as well as in the pneumonotomy and they give a prussian blue reaction with potassium ferrocyanide Per se the detection of asbestos bodies does not necessarinlecyessarily imply either severe an indication asbestosis or diminished work capacity but it does give that the patient has a predisposition for the disease A few more remarks about asbestos needles They are capabloef exerting mechanical irritation When workers are engaged in asbestos mining or processing asbestos fibers often penetrate - singly or in bunches - into the skin of their palms and the flexor side of their fingers This causes an epidermal proliferation called asbestos wart I have found such warts at whose center there is always an asbestos fiber on the skin of upper thighs and waists and I have discussed these cases eight years ago at a congress for internal medicine in Berlin But the warts formed by the proliferation and induration of the skin's epithelial layer are harmless it is the mechanical irritation which asbestos needles can provoke in lung tissue that is much significant We are then faced with neoplastic growth There is a- fortunately no longer any doubt that lung cancer is the occupational canceorf asbestos workers in the last three years 1935-1938 six different authors have reported 14 cases of combined asbestosis and lung cancer And considering the very limited number of autopsies carried out so far the percentage of cancer cases is very high Nord- mann in Hanover has assembled the pertinent literature and reportedly A, recently 8th International Congress on Occupational Diseases Frankfurt ) on the occupational cancer of asbestos workers the 18 years between " " " the beginning of work on asbestos and death match other occupational cancers Since there is no absolute effect occupational connection between silicosis and cancer it is clear that asbestos dust must act differently than silicosis dust . A few remarks now on the combination of tuberculosis and as- bestosis In England tuberculosis is really not rare Wood and Gloyne identified it in 30 of their asbestosis patients Merewether reports that in 1934 out of 27 asbestosis deaths 15 had tuberculosis for 7 19 Bridge even reports 34 tuberculosis cases amont 59 deaths from asbestosis but in Germany there have up to now only been isolated cases altogether 12 cases of asbestosis plus tuberculosis have been reported Indemnity liability under the Third German Decree on Occupational Diseases is therefore limited to severe asbestosis not combined with tuberculosis Nor of course is the combination asbestosis plus tuberculosis listed as yet However insurance companies have unhesi- tatingly recognized and idemnified as occupational disease even wild asbestosis with fatal lung cancer It appears certain that in order to contract asbestosis or sili- cosis there must be an underlying predispostion Whether a particular constitution such as a particular type of nasal breathing has a sig- nificant effect cannot yet be determined The chairman of the Silicon Committee Department counselo Drr. Bauer has placed instrumentation at the disposal of the committee on Asbestos for the purpose of evaluating the Lehmann theory on nasal capacity for dust absorption so far the tests given to 100 asbestos workers have not confirmed this hypothesis Py = BPN 55)ag: 794.
