(Conoco; Interoffice Communication To R. W. Seymour From R. Collins Date October 9, 1975 Subject VCM RELEASE At 1415 on 10/9/75, there was a VCM release at the large reactor area. Cause of VCM Release: VCM Charge Valve Leaking Joe Gann, Maintenance man, was in process of removing skillet from D-300 charge manifold and detected VCM coming out of line. Joe was wearing a fresh air mask and rubber gloves at time. He then moved away from job site and reported leak to Bud Stone, Field Operator. Bud then radioed Bobby Chaffin, Panel Operator, and reported leak and location to him. Bobby, at this time, tried to set deluge system off manually from inside control room, but could not. He then instructed Sammie Stokes, Field Operator, by means of radio to set deluge system off manually at deluge house. Sammie was at V-ll dryer room at time. He then put on a Scott Air Pac, located on walkway between V-ll dryer room and large reactor area, and set deluge system off. I instructed Bobby to put cold H20 to suction of "A" charge H2O pump, in order to get H2O pressure on charge manifold and to help dilute VCM coming out of line and also to wet area to prevent sparks until the deluge system was set-off. Bobby then dialed vapor release. No. 62, then dialed P.A., No. 8, and announced location of vapor release. He then called the dryer control room to insure that the dryers had been shutdown. R. C. Towery, Temp. Shift Supervisor, had already shut the dryers down using the automatic shutdown switch. We let the deluge system run for approximately 15 minutes then shut off. Using Scott Air Pacs and buddy plan, Sammie Stokes and I checked area with OVA meter and found area to be vapor free. Still wearing the Scott Air Pacs, we then checked line where leak occured and found line to be depressured. DTH 000095854 4 R. W. Seymour October 9, 1975 VCM Release Page 2 We then removed skillet, replaced gasket and tightened flange bolts. We again checked area with OVA meter and found area still vapor free. Owen Tubb and Butch Bryan also checked surrounding areas with OVA meters and detected no VCM. I then instructed Bobby to dial the all clear. No. 63, so normal operations could resume.
