3. Yes - During the time when he was an Injection Holding Machine Operator. Injection molding machines generate heat up to 650F. One resin drying oven was In this area with temperatures of 225F (with doors closed). The petitioner's work station was approximately 40 feet from the oven.

B. During the start-up of an Injection molding machine, .a small amount of decomposition products from the particular thermoplastic resin being used Is emitted.
C. Approximately 5-6 minutes a start-up, two to three times a week. The petitioner worked In this limited exposure for five years.

Injection Holding Room - 4 celling fans emitting through roof to the outside atmosohere. Also two archways and large overhead door for Inlet air.
Automatic Slicing Room - 3 archways, 2 - 30", 1 - 36"

9. Yes - Such samplings of the facility air were made In the past (specific dates not available) by the sampling of the air being exhausted through our blower systems by our then current Insurance carrier. Employer's Insurance of Wausau, and later by an EPA subcontractor. In neither case was
there noted to be any trace of harmful materials or Irritants.
More recently an Industrial hygiene survey, was performed to
evaluate employee exposure- to asbestos In the manufacturing
of gaskets. This survey which was performed by a Senior
Industrial Hygienist of our current Insurance carrier, Aetna

A Spiral Wound Machine Operator's basic task would be to
set up the machine with the proper template. The template
would be attached to the rotating spindle on the machine.
He would then begin to spirally wind a given length of a
ribbon of steel (1/4" wide X .007" thick) and a like amount
of a ribbon of millboard asbestos (1/4" wide X .015" thick).
On completion of the finished size, he would apply three
spot welds and remove finished oroduct from template. He
would repeat this orocess during his work day at an averaqe
of 500 to 1000 production pieces.
An Injection Molding Machine Operator would make minor
adjustments to the molding tool, regulate temperatures
on molding tool and material chamber. Introduce raw
material Into feed hopper, open and close machine during
cycle, trim sprue and deflash finished part making It
ready for shipment.

Automatic Slicing Machine Set-Up Man was required to make
sure the parts feeder was properly aligned so that the parts feed to the machine on a continuous basis.- lie was also required to replace and sharpen the tool which cut the parts, and to adjust Its depth of cut. He also adjusted the spacing of the cuts so that parts of the proper dimensions

were produced. He was also responsible for the Inspection, and replacement, when necessary of the cutting mandrels uoon which the parts were machined. He was required to maintain records of production and .recards of machinery adjustments.

As I discussed with you previously, the spiral wound gasket product line and manufacturing facility was transferred from Garlock's Camden facility to Its Precision Seal Division facility In Gastonia, North Carolina during 1968. When the manufacturing facility was moved, It was moved llntact with all the equipment and Inventory loaded on trucks and shipped to North Carolina.
Within less than a week after arrival In Gastonia, the spiral wound gasket facility was back In full operation turning out finished product. Since then. It has been In operation without Interruption In the Gastonia facility being operated exactly as It was In Camden.
Since I can locate no reports of any measurements being taken of airborne asbestos fibers during the period the spiral wound equipment was being operated In Camden, I contacted our people at Gastonia and they assure me the equipment has not been modified In any way since It was transferred from Camden. With this In'mlnd, I requested copies of any asbestos exposure surveys that have been taken In their facility since 1968. In response to this, they have supplied me with copies of two (2) such surveys which In turn I am providing to you (copies enclosed).

In addition, we discussed the workmen's compensation claim of WJ Gross wherein he alleges injury due to inhalation of asbestos and Teflon as well as other Injuries. This claim was filed during 1979 and as per our conversation Is being handled by the compensation carrier. I am not enclosing a copy of this claim as you Indicated It was not needed at this time.

sure in terms of duration and concentration. Dr. Amdur further testified that decedent's death was due to '`metastasizing small cell bronchogenic carcinoma'1 not causally related to his employment and that there was no evidence of carcinoma until iJ-2-75. He admitted that the etiology of dece dent's tumor is unknown.
Upon review of the whole record the Majority of the Board Panel finds, basec on the probative medical evidence in the record, particularly the testimony of Drs. Amdur and Bennett; that there is no evidence of asbestosls, that th: decedent did not sustain an occupational disease within the meaning of the Law and that his death was not causally related to his employment.
Accordingly, the decision filed 12-23-80 is AFFIRMED. The claim is disallow and the case is closed.

This refers to Employers Mutual Letter of June 7, 1979, under the above reference.
Today (6/14) I contacted the following people:
Richard Calkins Charles Nichols Henry Ververs Peter Archbold
I advised them of the preliminary meeting on Tuesday, 6/19/79 at 1:00 P.M. in Palmyra; and of the second meeting on Thursday, 6/21/79 at 10:00 a.m. in Rochester.
The above employees will all attend the 6/19 meeting. All except C. Nicholswill attend the 6/21 meeting in Rochester. Mr. Calkins will be working the 2:00-10:00 [?] shift next week, but he will be at the meeting 6/19 at one.
Jack, you asked me to record the fact in this memo that you talked to Bill Bow this morning (6/14) in accordance with PSH's instructions yesterday. Bill said that we do not need the services of Mr. Chapman, our labor lawyer in this case. We never have used any outside counsel on workmen's compensation cases. Bill said he would advise PSH accordingly.

Dear Jack:
The above deceased eoployee applied for New York State Disability Benflts in Apirl of 1975 and that claim was handled by your Personnel Department. In September of 1975 Dr. Jack Resnick filed a supplemental report and diagnosed carcinoma of the lung, stating that it should be covered by compensation. It was on the basis of that report that ve index the above file and investigated the claim.
On October 13, 1975, this claimant filed a formal claim (C-3) with the New York State Workmen's Compensation Board for lung cancer and this cllam was controverted by us. There have been numerous hearings in the case during which we have maintained our controversy. Until recently the claimant's attorney had been unable to establish a prlma facia case lacking the necessary medical opinion on the question of causal relationship. Dr. Sidney Sobel's report of September 8, 1977 changed the status.
The Special Fund has now become involved under Section 15-8EE of the Workmen's Compenstaion lav. They obtained and served a subpoena on Carlock requiring all X rays and medical reports be made available at the Buffalo District office of the Workmen's Compensation Board. This is what Dr. Kapur has referred to. The Special Fund intends to have these records examined by their consultant.
Since this claim is still in the preliminary stages it is difficult to predict what the final outcome will be and what form it will taka.

Dear Jack,
At the outset, please refer to ay August 14, 1978 summary regarding this matter, which was in response to your July 20, 1978 inquiry. Since that summary there have been a number of hearings at which time, testimony was taken from Dr. John Bennett, an impartial specialist assigned by the Worker's Compensation Board to review the file for his medical opinion.
As a result of chat testimony, the presiding judge is desirous of developing the record regarding the claimant's work history and exposure at the plant. This case has been set for further testimony on these Issues on thursday, June 21, 1979 at 10:00 A.M. at the Worker's Compensation Board, 155 West Main Street, Rochester, New York 14614. The employer is being directed to produce Richard Calkins and Chuck Nichols. The employer is also bslng dlreccsd to produce the plant Safety Director with records pertaining to all tests performed in the Textile Department^ along with an industrial engineer who can describe the operation in which the claimant was involved. Your employment records will give you the dates involved.
