The following information was obtained verbally from Bill Glick unless otherwise noted.
Background: The Fohl Village property is a Manufactured Home Community (MHC) owned by UMH Properties, Inc. The property has approximately 324 lots for manufactured homes. At the time of the inspection, the owner estimated 2.5 individuals per occupied lot, there were approximately 258 occupied lots.

Process Description: The treatment facility is an extended aeration basin with an equalization tank, rapid sand filters and a UV disinfection system.
This report does not contain Confidential Business Information (CBI).
Staff Interview(s): Per Bill Glick, the certified operator, the sewer collection system is composed of clay pipe between 8 inch and 12 inch in diameter. The system is gravity to equalization basin at the wastewater treatment plant. There was an Inflow & Infiltration (I&I) study conducted approximately 5 years ago. No lining or rehabilitation projects have been completed since the (I&I) study. Bill is not aware of any constructed overflows in the sewer system, and has not observed any overflows occuring in the collection system. UMH was unable to provide a copy of the I&I study.
A constructed overflow from the equalization basin with a pipe to the same unnamed tributary as the permitted outfall was found and removed approximately one year ago. Since the equalization basin overflow pipe was removed, there have been overflows from the headworks which have been reported to Ohio EPA when they have been observed. UMH was unable to provide copies of these incident reports. Ohio EPA provided a list of dates when the incidents occurred.

Data Collected and Observations: There is a water treatment room for the drinking water system and the swimming pool. The room has a floor drain which drains by gravity to the sewer treatment plant. There was a plastic bulk tank with chlorine for the swimming pool as shown in photos 1 and 2. Leaks from this tank would allow chlorine to flow by gravity to the sewer treatment plant.
At the sewer treatment plant, there is an influent chamber upstream of the equalization basin. This location is shown in photo 3, and is the location of composite sampling for the influent station. Per Bill Glick, this is the location where overflows from the headworks will occur during rainfall. Also per Bill Glick, the overflow will flow to the west in the direction shown in photo 8. The unnamed tributary which receives discharge 001 flows along the west side of the treatment plant.
Two comminutors, shown wrapped in blue tarps in photo 4, were out of service at the time of the inspection.

There are three aeration basins at the treatment plant. During the inspection Bill Glick conducted a settling test. Photos of the start of the settling test are shown in photos 11 through 15. The end of the settling test is shown in photos 33 through 36, and were 35%, 20% and 25% for the west, middle and east basins respectively.
There was growth and corrosion on the weirs in the clarifier which caused uneven flow into the overflow trough as shown in photo 17.
The influent and effluent composite samples are collected using a composite sampler. The composite sampling equipment is owned by Bill Glick and was not onsite at the time of the inspection. Per Bill Glick the composite sampling equipment is programed to collect samples on a time-based proportion, and the continuous flow meter is not used during the sample collection process.
Two of the six lamps in the UV system were burned out at the time of the inspection. See photo 25.

Areas of Concern: 1. NPDES Permit No. OH0117731 Part III-11. UNAUTHORIZED DISCHARGES Per Bill Glick, during rainfall events, the facility experiences overflows from the headworks. The overflows occur at a structure at the facility headworks and flow downslope to the same unnamed tributary where Outfall 001 discharges. Ohio EPA provided a list of events in an email on June 4, 2024. a) 1/24/24 to 1/30/24; b) 01/09/24 to 01/12/24; c) 3/3/23 to 3/5/23; d) 5/7/22 to 5/10/22; e) 3/23/22 to 3/24/22; f) 3/7/22 to 3/9/22; g) 2/19/22 to 2/19/22.
UMH did not provide copies of those incident reports.
2. NPDES Permit No. OH0117731 Part II, F. Composite samples shall be comprised of a series of grab samples collected over a 24-hour period and proportionate in volume to the sewage flow rate at the time of sampling. Such samples shall be collected at such times and locations, and in such a fashion, as to be representative of the facility's overall performance.
Per Bill Glick, a 24-hour composite sampler is used to collect the influent and effluent samples. The composite sampler is set to be a time-based composite. The continuous flow meter is not used during the composite sampling activity.

All wastewater treatment works shall be operated in a manner consistent with the following: A. At all times, the permittee shall maintain in good working order and operate as efficiently as possible all treatment or control facilities or systems installed or used by the permittee necessary to achieve compliance with the terms and conditions of this permit. Proper operation and maintenance also includes adequate laboratory controls and appropriate quality assurance procedures...
a) There was no containment for a bulk tank of chlorine used for the swimming pool. Leaks from the bulk tank would flow by gravity to sewer treatment plant.
b) The comminutors and the alarm systems at the equalization basin were not fully functioning at the equalization basin at the time of the inspection.
c) Corrosion and growth on the clarifier weirs was causing uneven flow into the overflow trough. Pin flocs were visible in the clarifier and flowing over the weir.
d) Two of the six lamps in the UV system were burned out.
4. NPDES Permit No. OH0117731 Part III-6. Recording of Results
"For each measurement or sample taken pursuant to the requirements of this permit, the permittee shall record the following information: A. The exact place and date of sampling; (time of sampling not required on EPA 4500) B. The person(s) who performed the sampling or measurements; C. The date the analyses were performed on those samples; D. The person(s) who performed the analyses; E. The analytical techniques or methods used; and F. The results of all analyses and measurements." Part III-7. Records Retention "The permittee shall retain all of the following records for the wastewater treatment works for a minimum of three years except those records that pertain to sewage sludge disposal, use, storage, or treatment, which shall be kept for a minimum of five years, including: A. All sampling and analytical records (including internal sampling data not reported); B. All original recordings for any continuous monitoring instrumentation; C. All instrumentation, calibration and maintenance records; D. All plant operation and maintenance records; E. All reports required by this permit; and F. Records of all data used to complete the application for this permit for a period of at least three years, or five years for sewage sludge, from the date of the sample, measurement, report, or application."
Copies of the laboratory reports, chain of custody, permit application, current NPDES permit, supporting documentation for the UV disinfection system operation, and incident reports, were not provided and could not be reviewed for this report.

APPENDIX C: FIELD MEASUREMENT DATA AND SAMPLING
Jonathan Moody conducted field monitoring using an In-Situ AquaTroll 600 (AT600), Serial Number 1079129. The AT600 was equipped with a temperature/conductivity sensor, pH sensor, Rugged Dissolved Oxygen (RDO) sensor and a turbidity sensor. The chart below shows the results of the monitoring. The AT600 was calibrated May 28, 2024 using a 3-point pH calibration and the In-Situ "Quick Cal" solution for specific conductivity. The RDO sensor was calibrated using the 100% saturation method on May 29, 2024, just before starting the field monitoring.
